노인장기요양보험제도의 문제점과 개선방안에 관한 연구 (A Research on the Problems and their Improvement Plans regarding the Long-term Care Insurance System for the Elderly)

초록

지금 세계 대부분의 선진 국가들은 노인인구의 증가에 따른 고령화현상 때문에 자국의 사회복지 문제에 대한 여러 가지 정책적 방안을 마련하여 대처하고 있다. 우리나라도 예외는 아니다. 산업화 과정을 거치면서 사회적 변화가 나타나고 출산과 사망률이 낮아지면서 노인인구의 절대적･상대적 비율이 높아져 세계에서 가장 빠른 속도로 고령화가 진행되고 있다. 이러한 현상은 치매･중풍 등 노인성 질환에 따른 일상생활이 어려운 노인들의 수를 날로 증가시키는 결과로 나타나면서 그에 따른 사회적 위험성 또한 크게 증가하게 되었다. 이러한 사회적 위험구조는 다양하면서 장기간에 걸쳐 지속하게 된다. 특히 고령화 사회의 사회적 위험에 따른 노인의 장기요양문제는 의료비 증가와 더불어 노인 부양문제 등 한 가정의 문제를 넘어, 국가의 개입이 필요한 우리 사회가 시급히 해결해야 할 심각한 사회문제로 대두되고 있는 실정이다.
고령화에 따른 사회적 위험을 먼저 경험한 독일은 노인장기요양보호의 사회화를 통해 급속히 증가하는 장기요양에 정책적으로 대응하고자 사회연대원리에 기초한 사회보험방식의 수발보험을 제정하여 대처하고 있다. 일본 역시 급증하는 노인장기요양의 수요에 대한 대응책으로 기초지방자치단체인 시정촌(市町村)을 법적･공식적 복지정책 결정자로 규정한 분권지향적 사회복지제도인 사회보험원리와 사회연대성을 원용한 개호보험제도를 제정하여 대처하고 있다. 이 시점에서 제도시행 만 8년에 이른 우리나라의 노인장기요양보험제도의 공･과를 살펴보지 않을 수 없다. 그동안 많은 성과를 내면서 등급조정 등 혜택범위가 대폭 확대되면서 많은 수급자가 노인장기요양서비스를 받고 있지만, 사회적 변화에 따른 제도의 여러 가지 문제점이 나타나고 있는 것도 사실이다.
따라서 본 연구에서는, 제도를 시행하면서 문제점을 개선하고 개혁하는 독일과 일본의 제도와 비교분석하여 시사점을 모색하고 문제점을 도출하여 몇 가지 개선방안을 제시하고자 한다. 첫째, 재원조달방식의 문제에 대한 해결방안을 모색하여야 하고, 둘째, 요양관련 정보부재의 문제를 적극적으로 구체화시켜 나가야 한다. 이러한 개선방안은 수요자 중심의 서비스 개발과 제공을 통한 지속적인 제도의 유지와 발전을 위하는 길이고, 고령화 사회에 따른 사회적 위험에 대처하는데 그 함의를 두고자 함이다.

영어초록

Most of the developed countries in the world now are responding and coming up with various policies regarding social welfare within their own countries because of the increase in the aging population. Through the rapid industrialization process, social changes have occurred while birth and death rates have dropped drastically. With this turn of events, the absolute and relative rate of the aging population have sharply increased and one of the world’s fastest aging phenomenon is happening in Korea. This phenomenon has resulted in intensifying the number of elderly people on a daily basis who find it difficult to maintain normal lives because they are suffering diseases such as dementia and strokes. As such, the social dangers followed by the aforementioned situation has also risen in multitudes. This kind of social and dangerous structure will inevitably continue on a long-term basis in a variety of ways. The long-term care issue of the elderly followed by the social dangers of the aging society is especially serious along with the growth in medical costs and this is not just a matter of one family taking care of its own senior member. In fact, the magnitude of the problem is not in single units, but is in imperative need of the attention of the state in a much grander scale and it is indeed a desperate situation where the society must resolve this matter urgently.
Germany has already experienced these dangers of the social aging phenomenon and is responding by enacting a kind of a social insurance method care policy based on the principles of social solidarity to fight against the rapid increase of the elderly people who need long term care through the socialization of the long term care protection system. Japan is also countering the demand for long term care for the rapidly surging number of the elderly population by enacting the long term care insurance system which has been derived from the social insurance principle and social solidarity which is a decentralized tendency social welfare system. Under this system, it places the basic local autonomies, the municipalities of Japan, to be the decisive policymakers in the legal and official capacity.
At this point, we cannot help but take a closer look at the results and problems of Korea’s long-term care insurance system for the elderly people which has been in execution for the past 8 years since its enaction. It is true that there have been various successes with the drastic expansion in benefits in terms of modification of the classification level of the elderly in their access to the long term care but it is also true that many problems have been surfacing within the system due to the social changes.
Therefore, attempts have been made in this paper to come up with a few improvement plans by rooting out the problems and coming up with focal issues by comparatively analyzing the systems of Germany and Japan which are both continuously carrying out reforms. First, a solution must be sought out for the problem of the resource procurement method. Second, the problem of lack of information related to the care system must be actively specificized. These kinds of reforms will be the path to the maintenance and improvement of the system through the development and provision of a customer centered service. There will also be a great significance in this in terms of responding actively to the social dangers of the ever aging society.
