However, it lacks definitional consensus and conceptual clarity. An explicit, empirically based model of case management is presented on the basis of 15 functions and their dynamic interrelationship along a sequential dimension. At the same time, nobody quite knows what case management is. This article proposes an explicitly defined model of case management practice. Rubin (1987) traced the antecedents of case management to vocational rehabilitation, public health nursing, and social work.
