헬스 리터러시(Health Literacy) 개념은 1974년 보건교육의 세부 목표로 처음 언급된 이후[1], 1990년대 후반부터 국가 건강증진의 핵심 전략으로 주목받았다[2]. 세계보건기구(WHO)는 1986년 오타와 건강증진선언에서 개인과 조직의 헬스 리터러시 역량 강화를 강조했으며, 2016년 상하이 선언문에서는 헬스 리터러시를 건강 형평성과 건강증진의 중요한 수단으로 정의하고, 이를 정책적 수준으로 확대할 필요성을 제기했다[3]. 2000년대 이후 여러 나라에서 국가 차원의 헬스 리터러시 조사가 시행되었으며, 유럽, 미국, 호주, 중국 등이 주요 국가로 참여했다[4]. 특히 중국은 2008년 첫 조사를 시작으로 헬스 리터러시를 보건정책인 Healthy China 2030에 포함시켰다[5][6][7].
