연장입양 아동과 주 양육자와의 애착형성을 위한 음악중재 사례 (A Case Study of Music Intervention for Attachment Formation between Extended Adoption Children and their Primary Caregivers)

초록

본 연구는 연장입양 아동과 주 양육자의 안정적 애착관계형성을 위한 음악치료 사례연구이다. 연장입양 아동이란, 임시적인 돌봄 환경에서 지내다 태어난 지 12개월이 지나 입양된 아동으로 본 사례에서는 연장입양 아동과 주 양육자와 3쌍을 대상으로 2021년 11월-12월 주 2회 40분씩 총 8회기 음악중재를 제공하였다. 연구자는 선행연구와 자료조사를 토대로 음악중재 프로그램을 개발하였다. 프로그램은 총 3단계로 구성되었으며 프로그램의 단계는 애착척도(IPPA-R)의 하위요인을 중심으로 구성하였다. 애착행동관찰을 위한 애착평가 도구는 Q-set의 하위요소 질문을 참고하여 연구자가 음악적 행동으로 수정한 후 음악치료 박사 2인에게 타당도 검증을 받아 사용하였다. 아동과 주양육자와의 애착변화를 분석하기 위한 자료의 수집은 부모-유아 애착척도 사전-사후 검사 및 아동애착행동 변화를 분석하였다. 참여 아동들의 애착행동은 5가지 하위 영역으로 자료 수집하고 분석하였으며, 음악적-비음악적 애착관련 변화를 정성적으로도 기술하였다. 연구결과 부모-유아 애착척도 사전-사후 검증에서 참가자 전체 평균 5점이 향상되었다. 참여아동의 애착행동의 변화는 순응, 도움요구, 안전기반행동, 정서공유, 신체접촉의 5개 하위 영역에서 전반적으로 상승하였으며 그 중 신체접촉과 도움요구하기 항목에서 가장 큰 상승 폭이 있었다. 또한 애착행동 변화에 대한 음악적-언어적 정성적 평가 결과는 음악중재가 입양연장아동과 주 양육자에게 애착형성을 위한 긍정적인 장(場)이 되었음을 보여준다. 입양연장 아동들은 음악적 환경에서 안전한 대상과의 상호작용 경험을 제공받았으며, 주 양육자들은 음악활동을 통해 양육 태도와 정서적 조율의 방법을 습득하고 연습할 수 있었다. 본 연구는 연장입양아-주 양육자 대상 국내 첫 음악중재 사례로 그 의의가 있으며, 추후 연장입양 아동과 주 양육자가 상호 건강한 애착관계를 형성하도록 돕는 음악중재 및 연구의 기초가 되었다고 사료된다.

영어초록

This is a case study of music therapy for the development of a secure attachment relationship between extended adoption children and their primary caregivers. Extended adoption children refer to children who have spent at least 12 months in temporary care before being adopted. In this case study, three pairs of extended adoption children and their primary caregivers were provided eight sessions of music intervention sessions for 40 minutes, twice a week, from November to December 2021. The researcher developed the music intervention program based on prior research and literature review. The program consisted of three steps based on the sub-factors of the Inventory of Parent and Peer Attachment-Revised version (IPPA-R) attachment scale. For attachment behavior observation, the researcher modified the Q-set sub-factors into music-related behaviors, which were then validated for reliability by two music therapists. Data were collected to analyze changes in attachment between the children and primary caregivers using the attachmentscale pre- and post-tests in children attachm ent behavior. The children's attachment behaviors were collected and analyzed in five sub-domains, and changes in both music-related and non-music-related attachment behaviors were qualitatively described. The results showed an overall improvement in attachment as an average score on the Parent-Child Attachment Scale increased by 5 points after intervention. The children's attachment behaviors improved in the sub-domains of compliance, help-seeking, secure-base behavior, emotional sharing, and physical contact, with the most significant increase observed in physical contact and help-seeking. The qualitative evaluation of music-related and verbal behaviors indicated that music intervention provided a positive environment for attachment formation between extended adoption children and their primary caregivers. The children experienced secure interactions with caregivers in the musical environment, while caregivers acquired and practiced parenting attitudes and emotional regulation strategies through music activities. This case study holds significance as the first music intervention study targeting extended adoption children and their primary caregivers. It provides a foundation for future music interventions and research to promote healthy attachment relationships between extended adoption children and primary caregivers.
