일본 개호보험제도의 2015년도 개정안 분석을 통한 정책적 시사점 (Analysis of the Contents of Revision of the Japanese Long‐term Care Insurance System in 2015 and Their Implications for Korean Policies)

초록

본 연구는 우리와 제도적으로 유사한 체계를 갖추고 있는 일본 개호보험제도의 운영 현황과 그간의 제도개선 과정, 2015년 개호보험제도 개정 내용을 분석하여 우리에게 주는 정책적 시사점을 제시하는데 목적을 둔다. 이를 위해 각종 선행연구 결과와 일본의 언론보도자료, 후생노동성이 제시한 정책 자료를 분석하여 1)일본 개호보험제도 현황, 2)그간 개호보험제도 개혁추진과정, 3)2015년도 개호보험제도 개정의 주요 내용, 4)한국에의 정책적 시사점 제시에 초점을 맞춰 다뤄보았다.
개호보험제도의 2015년도 개정 내용은 크게 지역포괄케어시스템 구축을 위한 재가의료체계 활성화와 개호 서비스와의 연계, 예방급여 재편, 개호노인복지시설 입소기준 제한과 소득수준에 따라 보험료 산정비율을 높이는 등의 비용부담의 공평화로 구성되어있다.
이러한 노력은 노인인구 증가에 따른 서비스 욕구를 충족시키면서 개호보험제도의 재정 부담에 효율적으로 대처하기 위한 방안이라고 볼 수 있으며, 개정내용 검토를 통해 우리 사회도 효과적인 의료-요양서비스 연계를 통한 지역단위 돌봄체계 구축과 함께 예방시스템 도입을 위한 단․장기적인 정책추진 방안을 고려해 나가야 한다.

영어초록

The purposes of this study was to analyze the operation of the Japanese long‐term care insurance system, which is similar to the equivalent system in Korea, past improvements of the system, and the contents of revision of the long‐term care insurance system in 2015, and to find implications for Korean policies. For these purposes, this study reviewed previous studies, reports of Japanese mass media, and policy materials published by the Japanese Ministry of Health, Labor and Welfare, and discussed: 1) the current state of the Japanese long‐term care insurance system; 2) the process of reform of the long‐term care insurance system; 3) major contents of revision of the long‐term care insurance system in 2015; and 4) implications for Korean policies.
The contents of revision of the long‐term care insurance system in 2015 consist largely of the promotion of community‐based medical care for the active establishment of regional comprehensive care systems, connection of community‐based medical services to long‐term care services, reduction of prevention benefits, restriction on eligibility to enter an elderly long‐term care welfare facility, and the equal bearing of costs through insurance fee calculation based on income level.
These efforts are believed to satisfy the increasing need of services resulting from the expansion of the elderly population and to suggest solutions for the efficient distribution of the financial burden of the long‐term care insurance system. Through the review of the contents of revision, moreover, our society also needs to build regional care systems through effective connection between medical and care services and to promote short‐term and long‐term policies for introducing prevention systems.
