18세기 혜민서의 의학교육 연구: 『혜국지(惠局志)』를 중심으로 (A Study on Medical Education of Hyemin-seo(惠民署) in 18th Century -Focused on 『Hyeguk-ji (惠局志)』-)

초록

본 연구에서는 혜민서 관청지 『혜국지(惠局志)』를 통해 18세기 의학교육의 단면을 드러내고자 하였다. 연구결과는 다음과 같다. 첫째, 혜민서에서는 원적(原籍)을 기초로 하여 혜민서의 운영과 치료업무, 의학교육 등의 다양한 역할을 위한 직제인 녹직(祿職), 외임(外任), 교관(敎官) 등을 두었다. 이 중 의학교육을 중심으로 볼 때, 학습자로서 생도(生徒), 출신(出身), 전함(前銜) 등 원적에 수록된 모든 이들과 의녀(醫女), 교수자로서 의학교수, 훈도 등으로 구분할 수 있다. 둘째, 혜민서의 의학교육은 예비의관 선발, 양성교육, 정기평가와 맞춤교육의 3단계로 구성되었다. 예비의관 선발에는 생도입속과 의녀선발, 양성교육에는 생도고강과 의녀고강, 정기평가와 맞춤교육에는 녹시(祿試), 총민시(聰敏試), 권장시(勸獎試)가 확인된다. 각 단계는 일정한 기간을 두고, 공정하고 지속적인 평가를 하며, 높은 수준의 학습 요구사항을 보여준다. 이러한 철저히 능력에 기반한 혜민서 내의 직제체계는 생도의 입속과정에서 의원 가문이라는 특혜를 받았더라도, 이후 의관이 되기 위해서는 높은 강도의 학습과정과 이에 따른 능력이 겸비되어야 한다는 것을 보여준다. 셋째, 혜민서는 같은 의료관청인 전의감과는 비교적 동등한 위치에서 역할분담을 했으며, 내의원과는 상․하관계로 연결되어 있었다. 혜민서는 활인서 참봉, 심약, 관청약방 등의 의관을 임명할 때, 전의감과 동등한 입장에서 교대로 차정했다. 한편 의녀를 교육하여 재예를 이룬 뒤 내의원으로 승차시키는 모습과 혜민서 최고의 의관이 내의원으로 추천되는 기록들은 내의원과의 관계를 짐작하게 한다. 또한 이러한 특징은 혜민서에서 의과가 별로 언급되지 않았던 이유에 대한 단서를 제공한다.

영어초록

This study attempted to reveal the characteristics of medical education in the 18th century through 『Hyeguk-ji (惠局志)』, a government journal of Hyemin-seo (惠民署). First, in Hyeminseo, there were The Administrations, The Local officers, and The Instructors for various roles such as Operation, Medical work, and Medical education of Hyeminseo based on Original Registration. learners and instructors can be distinguished by focusing on the function of medical education. Learners include Student, Successful candidates, Experienced men, and Medical women, nd instructors include Medical professor, and Medical trainer. Second, Hyemin-seo’s medical education consisted of three stages: preliminary medical doctor selection, training program, regular evaluation, and customized education. Preliminary medical doctor selection includes student admission and medical women selection, training program includes student’s oral test, medical women’s oral test, and regular evaluation and customized education confirmation for administration’s selection, examination for talented young men, examination for high ranking officers. Each stage has a certain period of time, conducts a fair and continuous evaluation, and shows a high level of learning needs. The system within Hyemin-seo, based on this thoroughly ability, shows that even if a person receives preferential treatment as a member’s family in the admission process of students, he must have both a high-intensity learning process and ability to become a medical doctor in the future. Third, Hyemin-seo divided roles in a relatively equal position to Jeonui-gam (典醫監), the same medical office, and was connected to Naeui-weon (內醫院) in a top-down relationship. When appointing some medical doctors, Hyemin-seo alternately selected and dispatched them on an equal footing with Jeonui-gam. On the other hand, the regulations on educating medical women, achieving achievements, and then promoting them to Naeui-weon, and the records of the best doctor Hyemin-seo being recommended as Naeui-weon, give us an idea of the relationship with Naeui-weon. These characteristics also provide clues as to why Ui-kwa (醫科) was not mentioned much in Hyeguk-ji.
