중국 민간중의사 자격 준입제도의 고찰 (Discussion on the Qualification Access System of Civil Traditional Chinese Medical Doctors in China)

초록

중국 민간중의사는 중의사팀의 중요한 부분으로 방대한 규모의 대오와 독특한 지리적 우위로서, 국가의 의료위생사업과 국민의 건강에 많은 공헌을 하였다. 의과대학 교육시스템이 형성된 이후, 중의과대학 교육은 과학화·전문화· 표준화라는 특징을 통하여 대중에게 인정을 받고 있는 반면, 전통사승(师承)· 가전（家传)과 독학방식으로 양성된 민간중의사 체계는 타격을 받아 점차 하락하고 있는 상황이었다. 중국의 많은 농촌지역과 산간오지에는 인구가 많은것에 비해 의료자원이 심각하게 부족하였으므로, 국가 위생 부문은 의료자원의불균형을 해소할 수 있는 방안이 민간중의사라 판단하여 민간중의사에게 합법적인 자격을 취득할 수 있도록 하였다.
현재, 민간중의사가 합법적인 자격을 취득하는 방식은 두 가지이다. 첫 번째 방식은 의사자격 고시제도를 통하여 개업중의사 자격을 얻는 것이며, 두 번째 방식은 중의의술 확유전장(确有专长) 인원 의사자격 고과제도를 통해 의사자격을 취득하는 것이다. 자격고시 제도하에서의 민간중의사는, 자격시험 합격하면 학력중의사와 같은 업무권한을 취득할 수 있었기 때문에 의사자격에 대한 불공평한 현상을 초래하였으며, 의료지식 부족으로 의료분쟁이 발생될 가능성이 높았다. 따라서 민간중의사의 특성에 적합한 의사자격고과제도의 도입은전통중의학의 계승에 도움이 될 것이라 사료된다. 민간중의사자격고시제도를폐지하고 자격고과제도의 미비점을 개선한다면, 합리적이고 완벽한 제도체계로서 민간중의사의 발전을 촉진할 수 있을 것이며, 중국특색의 다층적인 중의사팀을 형성하여, 중의학과 국가 의료위생사업의 발전을 위해 더욱 많은 힘을이바지할 수 있을 것이라 기대한다.

영어초록

Civiltraditional Chinese medicaldoctor is an important part of the team of traditionalChinese medical doctor in China. With their huge team scale and unique geographicaladvantages, they have made indelible contributions to the national medical andhealth industry and national health. After the formation of education system ofmedical schools, the education of Chinese medical schools has been recognizedby the public for its scientific, professional and standardizedcharacteristics.The system of civil traditional Chinese medical doctors trained by traditional methodsof mentoring, family transmission, and self-study has been gradually declined.China’s vast rural areas and remote mountainous areas have a large populationand serious shortage of medical resources. The state health department realizesthat in order to alleviate the imbalance of urban and rural medical resources,they can only rely on civil traditional Chinese medical doctors who should be given legalmedical qualifications.
There are two main ways for civiltraditional Chinese medicaldoctors to obtain legal medical qualifications: one is to obtain thequalifications of traditional Chinese medical doctors through the qualificationexamination system, and the other is to obtain the qualifications of peoplewith expertise in traditional Chinese medicine through the qualificationassessment system.
Under the qualification examination system, civiltraditional Chinese medicaldoctors can obtain the same practicing authority as academic traditionalChinese medical doctors after passing the qualification examination. This isnot only unfair, but also easy to cause medical disputes, because civiltraditional Chinese medicaldoctors are lack of theoretical knowledge. In contrast, the qualificationassessment system is more suitable for the development characteristics of civiltraditional Chinese medicaldoctors and is conducive to the inheritance of traditional Chinese medicine. Therefore, the author proposes to abolish the qualification examination systemfor civil traditional Chinese medical doctors, and at the same time modifythe imperfections in the qualification assessment system. It is hope to promotethe development of civil traditional Chinese medical doctors with a more reasonable andcomplete system, and form a multi-level traditional Chinese medicine doctorteam with Chinese characteristics, and contribute more to the development oftraditional Chinese medicine and national medical and health undertakings.
