과제보고서생 활 속 의 면 역과제보고서과제제목대상포진대 상 포 진1. 대상포진이란?대상포진은 바이러스성 피부질환의 일종으로 대상포진-수두바이러스에 의하여 신경에 염증이 생기는 병이다. 어릴 때 수두에 걸렸던 사람은 누구든지 대상포진이 생길 수 있다. 수두를 앓은 후 이 바이러스는 신경세포에 잠복해 있다가 신체저항력이 약해지는 경우에, 갑자기 증식을 하여 신경과 그 신경이 분포하는 피부에까지 통증을 수반한 염증이 생기게 되는데 이를 대상포진이라고 한다, 어린이들도 대상포진에 걸릴 수 있지만, 나이에 따라 발생률도 증가하여 50대 이후의 성인들에게 나타나는 경우가 흔하며, 외상이나 스트레스가 유발인자로 작용할 수 있다. 암환자나 방사선 치료를 받고 있거나, 면역억제제를 쓰고 환자 등 면역이 저하된 환자들이 대상포진에 걸릴 위험이 더 높기는 하지만 평소에 건강한 사람이 대상포진이 걸렸다고 해서 걱정할 필요는 없다.20세 이하의 건강한 사람에게는 발병률이 낮아 인구 1000명당 0.4~1.6명에서 발생하나, 85세 이상의 노인에서는 인구 1000명당 5~11명의 발생을 보인다. 발병의 남녀 비는 같으나 대상포진 후 신경통의 경우 남자가 여자보다 1.6배 많이 발생한다고 한다.2. 원인대상포진은 수두를 앓은 후 ‘수두 바이러스(Varicella-zoster virus)’가 완전히 소실되지 않고 우리 몸의 신경 절에 잠복하여 있다가 몸이 피곤하고 저항력이 감소된 틈을 타고 바이러스가 다시 분열하여 신경분포에 따라 피부감염을 일으키는 것이다. 피부신경에 침입하여 염증을 일으키고 신경을 파괴시키기 때문에 전기가 통하거나 바늘로 찌르는 듯한 통증을 유발한다. 또한 지속적인 관절염 통증과는 달리 아프지 않다가도 뜻하지 않게 갑자기 통증이 오며 시간적인 사이를 두고 불규칙하게 칼로 찌르는 것처럼 아프기 때문에 특히 참기 힘든 통증이다.모든 나이에서 발생할 수 있지만 대부분 노인들(40~60)에서 발생한다. 극히 일부의 환자들에게서 대상포진 감염이 재발하는 것으로 알려져 있으며, 수두와 같이 전염성이 있고 공기감염의 경로를 통하여 수두감염이 될 수 있다. 호발부위는 흉부(55%), 뇌신경(20%, 삼차신경에 가장 호위), 요추신경(15%), 천골신경(5%)순이고, 주로 세포성 면역의 장애가 있는 경우에 잘 발생한다.3. 증상대상포진이 걸리면 우선 몸의 좌우 중 어느 한쪽으로 일정한 부위가 아프거나, 따갑거나, 가렵게 됩니다. 그런지 1-3일 만에 빨갛게 반점이 돋는다. 경우에 따라 열이 나거나 머리가 아플 수도 있다. 반점은 여러 개의 물집이 모인 모양으로 변한다. 물집은 차차 고름이 잡히다가 딱지가 되어 떨어진다. 정상적인 사람은 병변이 2-3주 후에 없어진다. 통증은 피부가 다 나은 후에도 오래 지속될 수 있다. 드물게는 물집이 없이 아프기만 하거나, 물집이 생겼는데 안 아픈 경우도 있을 수 있다. 대상포진은 주로 몸통과 둔부 쪽에 잘 나타나는데 얼굴, 팔, 다리 등 어떤 부위에도 나타날 수 있다. 대상포진이 귀에 발생하면 안면신경이 마비되어 입이 돌아가는 경우가 있다. 대개는 어느 정도 시일이 지나면 저절로 회복되지만, 급성기에 치료를 잘 받는 것이 좋다. 재발하는 경우는 드물며 재발 시 대부분 단순포진 바이러스 감염인 경우가 많다. 민감한 사람의 경우 대상포진을 가진 환자의 수포에서 배출된 액체와 접촉하기만 해도 수포가 발생할 수 있다. 그 전에 수두를 앓았던 사람은 면역이 있으므로 환자와 접촉해도 위험성이 없다.가) 전구증상- 대상포진의 첫 번째 증상은 통증과 이상감각이다. 피부발진보다 며칠 전에 나타나고, 피부 가려움, 얼얼한 느낌, 작열통, 천자통, 전격통의 형태로 나타나기도 한다.나) 피부발진- 침범한 하나의 지각신경분포를 따라 띠 모양으로 나타나며 일측성이고 중앙선을 넘지 않는다. 처음에 홍반위에 군집한 수포가 생기며 약 3일째 수포는 농포에서 7~10일째 가피로 된다.- 수포의 일부는 터지지 않고 그대로 마르고 일부는 출혈성 또는 괴사성으로 되며 악성 질환을 앓고 있는 사람에서는 대수포가 생기기도 한다.다) 통증- 감염된 신경에 해당하는 부위에 피부발진보다 먼저 시작된 통증이 심해지고 지각과민, 통곽과민 등의 증상이 나타나며 의복과 접촉 시 통증의 강도가 증가한다.라) 범발성 대상포진- 쇠약한 노인, 악성종양, 후천성 면역결핍증 환자등에서 대상포진이 신경분절을 따라 나타난 후 전신에 수두형 발진이 생기는데 가끔 이들 수포는 출혈성이나 괴사성일 수 있고 발열, 두통, 수막 자극 증상이 나타날 수 있다.마) 안구대상포진- 제 5 뇌신경의 안신경 부분이 침범된 경우로 각막의 수포 및 궤양으로 실명할 수도 있다.바) Ramsay Hunt 증후군- 수두 바이러스가 안면신경 및 청신경을 침범함으로써 동측에 안면마비, 귀앓이, 안구진탕증이 나타나는 것을 말한다.사) 천골 대상포진(Sacral zoster)- 제 3천골 신경 절, 드물게 제 2, 제 4 천골 신경 절에 대상포진이 생기는 경우 신경원성 방광이 발생할 수 있다.4. 합병증1) 치료된 병변에 나타나는 감각과민, 계속적인 통증, 작열감 혹은 간헐적으로 쿡쿡 쑤시는 통증이 특징인 만성 통증증후군(헤르페스 후 신경통)1) 3차 신경에서 눈과 연결된 부분의 침범으로 각막염, 포도막염, 각막궤양, 실명의 가능성이 있다.2) 안면과 청신경침범으로 청력손실, 현기증, 안면근육신경 약화 등을 초래한다.3) 내부기관으로의 전파 - 폐렴, 식도염, 장염, 심근염, 췌장염5. 치료치료의 목적은 통증완화와 합병증(감염, 상흔 형성, 신경통) 예방에 있다. 항 바이러스제를 가능한 빨리 투여하되 증상에 따른 치료도 병행한다. 대상포진은 다른 사람에게 옮겨 주는 전염력이 아주 약하므로 격리 치료할 필요는 없다. 몸의 면역력 약화나 스트레스 때문에 발생하므로 휴식 및 안정을 취하는 것이 중요하며 젊은 사람은 일상적인 활동을 하여도 좋으나 중년 이후에서는 육체적 활동을 피하고 안정을 취하는 것이 예방에 도움이 된다. 국소 열 찜질이나 통증에는 아스피린, 코데인 등 진통제가 효과적이다. 특히 눈에 물집이 생길 때는 후유증으로 시력장애가 생길 수 있으므로 주의해야 하고 안과에서도 진료를 받아야 한다. 피부증상이 다 나은 다음에도 지속적으로 또는 간헐적으로 통증이 지속하는 경우를 포진 후 신경통이라고 한다. 나이가 드신 분일수록 신경통이 오래(때로는 수년간) 남는 경우가 있다. 면역이 저하된 환자의 경우에는 수두와 같이 전신에 물집이 생기는 경우도 있다.
