1. 뇌졸중의 일차예방
뇌졸중은 사망률이 높은 질환일 뿐 아니라 생존한다하더라도 그 후유증이 심각해 개인적으로 그리고 사회적으로 커다란 부담을 남기게 한다. 전체 뇌졸중의 75%는 이전의 뇌졸중의 과거력이 없는 환자들이고 또한 뇌졸중으로 인한 후유증과 경제적 손실 등을 생각한다면 뇌졸중의 위험인자들을 가진 환자들에서 뇌졸중이 발생하지 않도록 하는 것이 중요하다.
이러한 뇌졸중의 일차적 예방을 위해서는 뇌졸중의 유발과 연관된 위험인자들을 이해할 필요가 있다. 뇌졸중의 유발인자들은 교정이 가능한 것들과 교정이 불가능한 인자들로 나누어 질 수 있는데, 이러한 뇌졸중의 유발 인자들 중 연령이나 성별 혹은 인종 등과 같은 교정이 불가능한 것들을 제외하고 교정이 가능한 여러 가지 위험 인자들과 그 영향에 대해 인지하고 이에 대한 철저한 대비가 필요하다 (Table 1).


1.1 뇌졸중 발생에 관한 위험에 대한 분석
뇌졸중의 발생 가능성에 대한 평가방법이 적절하게 이루어질 수 있다면 뇌졸중 예방에 큰 도움을 받을 수 있다. 특별히 뇌졸중의 위험인자들은 각기 독립적인 인과관계를 가지고 있지만 서로 밀접한 연관성을 가지고 뇌졸중의 발생에 영향을 미치므로 이에 대한 정확한 분석이 필요하다. Framingham Stroke Profile (FSP)는 연령, 고혈압, 당뇨, 그리고 여러 가지 뇌졸중 위험인자들의 존재 유무와 그 정도를 통계적인 방법으로 분석하여 점수화시킨 것으로 개인의 뇌졸중 발생에 대한 가능성 평가를 가능하게 한다 (Table 2).


1.2 고혈압의 조절
고혈압은 뇌경색과 뇌출혈 발생에 밀접한 연관을 가진 인자로 혈압이 높을수록 뇌졸중의 발생율은 비례하고 수축기 혈압을 10 mmHg이상 감소시킬 때마다 뇌졸중의 발생빈도가 의미 있게 감소한다. 따라서 고혈압 환자에서 혈압약의 사용은 뇌졸중의 발생빈도를 35-44%까지 줄인다고 알려져 있다.최근에 제시된 고혈압 환자에서의 목표치는 140/80mmhg이며 당뇨가 합병된 경우에는 더 낮게 유지할 것을 권장한다.
