ADHD 아동·청소년의 진단평가 절차에 대한 고찰 (A Review of Diagnosis and Evaluation Procedure for the Child & Adolescent with Attention Deficit Hyperactivity Disorder)

초록

본 연구에서는 주의력결핍 과잉행동장애(ADHD)의 의료적 진단평가 절차에 대한 교사와 부모들의 이해를 돕고 ADHD 진단평가에 기여할 수 있도록, 미국소아과학회(AAP)의 진단지침을 중심으로 아동·청소년의 ADHD 진단 과정과 원칙, 선별과 평가, 진단 결정을 포함한 진단평가 절차를 고찰하였고 다음과 같이 제언한다. 첫째, 정확하고 안정적인 진단을 위해서 진단 원칙을 준수하고 진단 알고리듬을 통해 체계적으로 접근해야 한다. 둘째, 선별과 평가를 위해 부모를 포함한 가족과 교사와 지역사회 주요 관련인 그리고 아동 본인으로부터 타당한 절차와 도구를 통해 효과적인 정보를 수집해야 한다. 진단뿐만 아니라 치료 및 중재를 위해서 아동과 부모, 교사 및 학교와 긴밀한 관계형성 노력이 필요하다. 셋째, ADHD 진단은 DSM-5 진단기준의 부합 여부 확인, 연령과 성별에 대한 고려, 동반질환 파악과 다른 질환의 배제 과정을 거쳐 결정해야 한다. 또한 진단을 결정할 때 부모의 기대와 신념, 정신질환에 대한 인식도 고려해야 하며, 미처 파악하지 못한 요인들이 아동의 현재 문제를 형성할 수도 있다는 점을 유념해야 한다. 마지막으로 보다 정확한 진단을 위해서 관련 학회나 협회 차원에서 ADHD 진단지침을 개발하고 이를 임상의들에게 지속적으로 교육해야 하며, ADHD를 포함한 아동기 정신질환과 DSM-5의 진단기준을 우리 문화와 사회, 보건의료체계에 맞게 이해하고 적용하기 위한 관련 분야의 초학문적 연구 노력이 필요하다.

영어초록

The purpose of this study is to help teachers and parents understand the medical diagnostic evaluation procedure of ADHD and contribute to more accurate ADHD diagnostic evaluation. To this end, we reviewed the diagnostic guidelines, including the medical diagnosis process and principles, screening and evaluation, and diagnosis decisions for ADHD in children and adolescents, with a focus on diagnostic guidelines from the American Academy of Pediatrics (AAP). According to the results, we suggest the following. First, in order to make accurate and stable diagnosis, it is necessary to be adhered the diagnostic principles and systematically diagnose it through a diagnostic algorithm. Second, for screening and evaluation, it is necessary to collect effective information through appropriate procedures and tools from parents, teachers, important community informants, and children themselves. In addition to diagnosis, treatment and intervention require close efforts to form close relationships with children, families, and teachers. Third, the diagnosis of ADHD should be determined by checking whether the DSM-5 diagnostic criteria are met, considering age and gender, identifying comorbid conditions, and excluding other diseases. Parents' expectations, beliefs and perceptions of mental disease should also be considered when deciding on diagnosis. It should also be noted that factors that have not yet been identified can shape the child’s current problems. Finally, for a more accurate diagnosis, it is necessary to develop guidelines for ADHD diagnosis at the level of related societies or associations, and to continuously educate them to clinicians. Also transdisciplinary research efforts in related fields are needed to understand and apply the diagnostic criteria for childhood mental disorders including ADHD and DSM-5 in accordance with our culture, society, and health care system.
