미국 병원의 의료 전문직종 역할분담에 관한 조직적 요인 - 의사, 간호사, 의료전문직(Non-Physician Clinician)을 중심으로 - (An Organizational Factors in the Role Sharing of Medical Professions in U.S. Hospitals Focusing on Doctors, Nurses, and Non-Physician Clinicians)

초록

현재 미국 의료전문직의 모습은 미국 특유의 의사, 간호사가 일하는 방법 및 그 의료전문직이 만든 병원조직을 나타낸 모습이라고 할 수 있다. 본고에서는 미국의 병원이 가지는 특징 가운데, 의료조직의 역할이 세분화되어 있는 것, 합리화·표준화가 진행되고 있는 것, 오픈 시스템이 취해지고 있는 것의 3가지가 어떻게 급성기 의료의 현장에서 의사, 간호사, PA/NP의 역할분담에 영향을 주었는지를 논의한다. 보험제도, 의료수가제도, 의료 종사자 수, 의사의 소득, 국민성 등 미국의 NP/PA 시스템 그대로를 한국에 도입하는 것은 절대 현실적이지 않다. 그러나 미국에서의 NP/PA제도의 실태를 정확히 아는 것은 우리나라에서의 현상을 개선하기 위한 힌트를 얻을 수 있고 그 밖에 향후 팀 의료 추진에 관한 논의에 이바지할 것으로 생각된다.
국내에서도 의료전문직 도입의 옳고 그름이 논의될 때에는 제도 자체를 창설함과 동시에 병원조직의 기본방향이나 의사, 간호사의 역할을 객관적으로 검증하고 의료전문직을 어떻게 우리나라의 병원조직에 맞는 형태로 정착시키고 활용시켜 나갈 것인지가 보다 중요하다고 생각한다. 팀 의료를 추진하기 위해서는 NP/PA제도 도입이 효과적인 수단의 하나라 생각되며, 현장의 요구, 역할분담과 도입할 경우의 장단점을 더 세밀하게 논의하는 것이 좋다고 생각된다. 관련 법률의 정비에 의한 재량권과 책임의 명시, 전문가 양성교육 체계 및 자격시험 기준의 제정, 의료 종사자와 대중의 이해 등이 추가 연구 과제이다.

영어초록

The current state of the medical profession in the United States can be said to represent the unique American doctors and nurses working methods and the hospital organization created by the medical profession. In this paper, among the characteristics of hospitals in the United States, three characteristics of the medical organization are fragmented, rationalization and standardization are in progress, and an open system is being adopted. Discuss whether PA/NP has influenced the division of roles. It is absolutely not realistic to introduce the NP/PA system of the United States, such as the insurance system, the medical fee system, the number of medical workers, the income of doctors, and the nationality, to Korea. However, knowing the actual situation of the NP/PA system in the US can give hints for improving the current situation in Korea, and it is thought that it will contribute to the discussion on the promotion of team medical care in the future.
When the right or wrong to introduce medical professions is debated in Korea, the system itself is established, objectively verifies the basic direction of the hospital organization and the roles of doctors and nurses, and how to establish and utilize the medical profession in a form suitable for the hospital organization in Korea. I think it’s more important to do it. In order to promote team medical care, the introduction of the NP/PA system is considered to be one of the effective means, and it is better to discuss in more detail the needs of the field, the division of roles, and the advantages and disadvantages of introducing them. Additional research tasks include the clarification of discretion and responsibility through the revision of related laws, establishment of professional training system and qualification test standards, and the understanding of health care workers and the public.
