일본 발달장애의 정의와 새로운 의료적 분류의 검토 (Review on the Japanese Definition and New Medical Classification of Developmental Disabilities)

초록

발달장애인의 교육권 보장을 위해서는 판별 준거 마련과 의학적 검토가 선결되어야 한다. 그러나 ‘발달장애’는 현재 널리 사용되는 용어이면서도 우리나라의 경우 명확하게 정의되어 있지 않은 개념이다. 일본에서는 판별 근거로 발달장애의 정의와 새로운 의료적 검토가 시도되고 있기에 일본의 발달장애 정의를 재검토하고, 일본 의료 분야에서 이루어지고 있는 발달장애 분류의 새로운 동향을 이해하는 것은 현재 우리나라 발달장애인 지원 및 권리보장에 관한 법률을 제정하는 데 기초자료가 될 것이다.
연구 방법으로는 최근 일본에서 간행된 정신신경소아과 의사, 아동정신의학자의 단행본, 당사자 저술의 단행본, 다양한 전문가의 의견을 수록한 책, 그리고 일본 발달장애학회의 연구지, 발달장해기본용어사전, 발달장해백서2008 등의 자료를 검토 분석했다.
연구 결과로 첫째, 발달장애는 당사자 측의 요소(본인의 성장, 발달에 동반한 변화)와 환경 측의 요소(주위와의 관계에 의해서 일어나는 변화)의 상호작용에 의해서 그 상태가 변화하는 장애로 발달상 조기 발증하고, 기능의 발달장애 또는 지체가 나타나며, 그리고 안정된 경과를 취한다는 특징을 알 수 있었다. 둘째, 아동학대가 발달장애로 새롭게 분류되고 있음을 알 수 있었다.

영어초록

The preparation of screening criteria and medical investigation are necessary for the guarantee of the right of education of people with developmental disabilities. Although developmental disabilities are one of the widely used terms, there isn't yet any clear definition of the term in our country. Japan attempts to define developmental disabilities and make a new medical investigation of the disabilities in order to prepare screening criteria. Therefore it's worth doing to look into the Japanese definition of developmental disabilities and a new trend in the classification of developmental disabilities in the Japanese medical sector, which is expected to provide useful information on the enactment of a law about support for people with developmental disabilities and the guarantee of their rights.
Data that were lately released in Japan were analyzed to serve the purpose, which involved books written by child neuro-psychiatrists and child psychiatrists, books by people with disabilities, books explaining the opinions of experts, the journal of the Japanese Academic Society of Developmental Disabilities, a glossary of developmental disabilities and a 2008 white book of developmental disabilities.
The findings of the study were as follows: First, there is a change in the state of developmental disabilities due to interaction between the personal factors(changes resulting from growth and development) and environmental factors(changes caused by relationship with others). As for the characteristics of developmental disabilities, this symptom takes place in the early stage of development, and that is concurrent with functional developmental disabilities or retardation and shows a stable progress. Second, child abuse is newly classified as one of developmental disabilities.
