한국 노인장기요양보험 수급조건의 국가별 비교평가를 통한 발전방안 연구 - 한국 · 독일 · 일본의 제도내용분석을 중심으로 - (A Study of an advancement scheme for the Korean Long Term Care System for the Elderly through Comparison and Evaluation of Its Eligibility Condition: Focused on the Analysis of Korean, German, and Jap)

초록

본 연구는 한국 노인장기요양보험제도의 도입을 앞두고 성공적 정착과 발전적 운영을 위한 정책적 고려를 탐색해보고자 이와 관련된 중요한 이슈로서 수급조건(eligibility condition)의 내용을 국가별 비교평가하고자 하였다. 즉 주요논의는 급여대상의 포괄성, 급여내용의 적합성과 적절성, 재정 및 운영의 효율성과 연관된 수급조건에 초점을 맞추되, 수급조건 분석내용의 포괄적 포함과 체계적 세분류, 제도내용분석의 심도 있는 검토, 국가별 비교평가라는 분석적 특성을 바탕으로 접근하고자 하였다. 이에 요양보험과 관련된 수급조건을 사회보험의 주요 수급조건으로서의 가입 및 기여조건과 사회적 위험(요양) 조건뿐 아니라 이의 적용과정에서 중요한 관련성을 갖는 수급권(자)조건, 개별급여수급조건, 병급조건, 수급 후 급여변동․제한조건, 수급조건 관리기구 등 총 7가지 항목으로 구분하여 살펴보았다. 분석 결과, 수급권(자)조건에서는 장기요양에 대한 명확한 개념규정과 특정연령 및 질병으로 한정한 문제, 가입 및 기여조건에서는 요양보험의 장기급여적 속성에 따른 최소가입기간조건설정과 본인부담제의 대안마련 문제, 요양상태조건에서는 요양의 양가적 속성을 제도 내에서 어떻게 조화시키는가와 관련된 문제, 개별급여조건에서는 수급자의 개별적 상황에 명확하고 유연하게 대응하는 것과 관련된 문제, 병급조건과 관련해서는 서비스 연속성추구를 위한 제도 내외적 관계설정에 초점을 맞추는 문제, 그리고 마지막으로 관리기구와 관련해서는 평가도구의 타당성과 평가조직의 전문성 확보문제 등을 주요 논의사항으로 확인하였고, 이에 대한 발전방안에 대한 논의가 이루어졌다. 이를 통해 제도발전의 함의도출에 문제제기로서의 역할과 실질적 대안마련에 풍부한 고찰을 제공하였다.

영어초록

This study examined policies for successful settlement and operation of the Korean long term care system for the elderly with the expectation that the government would introduce the system soon. It compared and evaluated eligibility condition of each nation, a significant matter related to the system. With the focus on eligibility condition related to comprehensive coverage, appropriate and adequate benefit, and efficient finance and operation, the study accessed the matter of eligibility condition based on comprehensive inclusion and systematic subclassification of analysis content of eligibility condition, in-depth examination of the analysis of the system content. and the analytical feature of comparison and evaluation by nation. Eligibility condition related to the long term care system was classified into the following seven sectors and examined as major eligibility condition of social insurance: entrance and contribution condition; social risk(care) condition; recipient condition which is important in the application of the system; individual benefit condition; concurrent condition; condition of benefit change and benefit limitation after receiving benefit; and organization managing eligibility condition. According to the analysis results, the following matters were confirmed as key issues and discussed: recipient condition- a matter of providing a clear definition of long term care and limiting ages and diseases eligible for the system; entrance and contribution condition- setting the condition of minimum entrance period attributed to the system's characteristics to offer benefit for a long period, a matter of providing an alternative to self-payment system; care condition- a matter of how to balance ambivalent characteristics of care in the system; individual benefit condition- a matter of responding precisely and smoothly to a recipient's individual conditions; concurrent condition- a matter of focusing on establishing relationships inside and outside the system to seek the continuity of service; and a management organization- a matter of securing verification of evaluation tools and professionality of evaluation organization. Through the discussion, questions were raised for the development of the system and efforts were made to produce an alternative.
