빠른 진행성 신부전을 보인 특발성 결절성 사구체경화증 1예 (A Case of Idiopathic Nodular Glomerulosclerosis Presenting Rapid Progressive Renal Failure)

초록

신 조직검사에서 뚜렷한 면역 침착이 없으면서 결절성 사구체간질 경화 (nodular mesangial sclerosis)와 사구체의 비대, 사구체 기저막의 비후가 관찰될 때, 이는 결절성 당뇨병성 사구체 경화증의 특징적인 소견으로 알려져 있다. 그러나 이러한 조직소견이 당뇨가 없는 환자에서도 보고되어 왔다. 1989년 Alpert와 Biava1)는 당뇨의 증거가 없고 면역 침착이 없으면서 결절성 사구체간질 경화가 있는 5예를 보고하면서 특발성 결절성 사구체간질 경화증 (idiopathic nodular mesangial sclerosis)으로 명명하였다. 또한 1999년 Herzenberg 등2)은 이러한 질환에 대해 특발성 결절성 사구체경화증 (idiopathic nodular glomerulosclerosis, ING)이라는 용어를 처음 사용하였다. 이후, 2002년 Markowitz 등3)은 특발성 결절성 사구체경화증 23예를 보고하면서 비교적 빠른 진행을 보이는 신부전과 심한 단백뇨가 나타나며 고혈압과 흡연이 병인에 중요한 역할을 하는 진행성 혈관병변 질환 (progressive vasculopathic lesion)으로 기술하였다.
저자들은 고혈압 진단 하에 항고혈압제를 복용해왔으며 당뇨의 병력이 없는 환자가 양 하지 부종과 크레아티닌의 상승으로 내원하여 신 조직검사에서 뚜렷한 면역 침착이 없는 결절성 사구체간질 경화 (nodular mesangial sclerosis)와 현저한 사구체 소엽화 (accentuated glomerular lobularity)의 병리 소견을 보였으며, 빠른 진행성 신부전을 보인 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

영어초록

Idiopathic nodular glomerulosclerosis is an unusual entity with histopathologic features resembling nodular diabetic glomerulosclerosis but occurs in non diabetic patients and predominantly in older males with smoking and long standing hypertension. It is characterized histologically by nodular mesangial sclerosis, glomerular basement membrane thickening, and arteriolosclerosis in the absence of immune-type deposit, and clinical presentations typically include renal insufficiency and severe proteinuria with relatively poor prognosis. We experienced a rare case of idiopathic nodular glomerulosclerosis presenting rapid progressive renal failure in a 42-year-old man without clinical evidence of diabetes. He was admitted with peripheral edema and renal insufficiency. He had a history of hypertension for 2 years and chronic hepatitis B for 5 years. Funduscopy and renal ultrasound were normal. HbA1c and oral glucose tolerance test were normal and on serum and urine electrophoresis, M-spike was not found. Light microscopic examination demonstrated the characteristic features of lobular glomerulonephritis showing glomerular hypertrophy and nodular mesangial sclerosis. Immunofluorescence was negative for immune-type deposits. Electron microscopy showed marked increase of mesangial matrix, mesangial cell proliferation, moderate effacement of foot processes, and glomerular basement thickening without electron-dense deposits or other specific fibrils. Idiopathic nodular glomerulosclerosis was diagnosed. After discharge, he continued smoking and his blood pressure was not controlled. Three months after first biopsy, second biopsy was performed because of increasement of serum creatinine and specimens showed progression of tubulointerstitial change and nodular mesangial sclerosis. Subsequent serum creatinine was progressively increased and hemodialysis was started 13 months after diagnosis. (Korean J Nephrol 2005;24 (1):313-319)
