자동차보험 대인배상 치료비 증가요인 분석 (Analysis on Korea Auto Insurance Bodily Injury Medical Expense)

초록

자동차보험 대인배상 치료비 증가세가 상해등급 12~14급의 경상환자를 중심으로 지속되며 도덕적 해이, 과잉진료 등 사회적 비용이 높아지고 있다. 감독당국은 경상환자 과잉진료 억제를 위해 다양한 제도 개선을 시행하였지만 그 효과는 미미한데, 감독당국은 아직 자동차보험 대인배상 치료비 증가에 영향을 미치는 요인을 찾지 못한 것으로 보인다. 본 연구는 2010년 1월부터 2021년 12월까지 시계열 자료를 이용하여 1인당 자동차보험 대인배상 치료비 증가율에 영향을 미치는 요인을 모색하기 위해 향후치료비와 한방 및 의과 외래진료비, 입원비 물가지수 등을 중심으로 분석한다. 향후치료비는 자동차보험 합의 후 발생할 수 있는 합리적으로 예측한 치료비로 합의금 역할을 하는데, 실제 발생하지는 않았지만 가해 운전자를 대위하는 보험회사가 관행적으로 피해자들에게 지급하고, 피해자는 향후치료비의 규모에 따라 치료 중단 여부를 결정하여 합의 시기를 앞당길 수 있다. 향후치료비가 합리적으로 예측된 치료비라는 점과 경상환자의 치료 중단 여부 결정에 영향을 줄 수 있다는 점을 추정방정식에 반영하여 2단계 추정방법으로 분석한 결과, 한방 외래진료비 물가상승률과 향후치료비가 경상환자 1인당 대인배상 실질 치료비 증가율을 통계적으로 유의미하게 확대시키는 것으로 나타났다. 이 결과는 자동차보험 경상환자의 과잉진료 등 사회적 비용 억제를 위해서는 향후치료비에 대한 관행 개선이 효과적일 수 있다는 점을 시사한다.

영어초록

Social costs such as moral hazard and excessive medical treatment are increasing along with the escalating bodily injury medical expenses in automobile insurance, particularly for patients classified with minor injuries ranging from grades 12 to 14. While supervisory authorities have implemented a range of policies to address excessive treatment for minor injuries, the effectiveness of these measures seems limited, indicating an inability to identify the underlying factors driving the surge in bodily injury medical expenses. This study examines time-series data from January 2010 to December 2021, analyzing future medical expenses, outpatient medical expenses for oriental medicine and surgery, as well as hospitalization price indices to discern the factors impacting the growth rate of bodily injury medical expenses per person in automobile insurance. Future medical expenses are reasonably expected medical expenses that may arise after accident closure and serve as settlement money. Victims may cease treatment and negotiate settlements based on the size of the future medical expenses, conventionally disbursed by insurance companies on behalf of the at-fault drivers, even before the expenses have been incurred. Employing two-stage regressions that consider the predictive nature of future medical expenses and their potential influence on settlements for patients with minor injuries, this research demonstrates that the inflation rate of outpatient expenses for oriental medicine and future medical expenses significantly contribute to increasing bodily injury medical expenses per patient for minor injuries. These findings suggest a need for reevaluation and modification of practices related to future medical expenses to mitigate social costs, particularly the excessive treatment of patients with minor injuries in auto insurance.
