마비말장애인의 조음오류 유형에 따른 말명료도 분석 (Analysis of Articulation Error Patterns Depending on the Level of Speech Intelligibility in Adults with Dysarthria)

초록

배경 및 목적: 본 연구에서는 말명료도 수준에 따른 마비말장애인의 조음오류 유형에 차이가 있는지 분석하고, 마비말장애인의 말명료도를 예측할 수 있는 조음오류 유형을 살펴보았다. 방법: 대상자는 화자와 청자로 나누었다. 화자는 뇌성마비를 동반한 마비말장애인 100명이며, 청자는 마비말장애인의 말을 청취해본 적이 없는 20대 정상 청력 성인 25명을 대상으로 하였다. 먼저, 신뢰로운 마비말장애인의 조음오류 유형의 분석을 위해서, 훈련된 평가자가 마비말장애인의 조음오류 유형을 APAC 녹음 자료를 사용하여 자음의 생략, 대치, 왜곡, 첨가, 음소 및 음절의 연장, 반복, 부적절한 쉼, 비구어적 소리를 분석하였다. 말명료도 측정을 위해서 청자가 조용한 환경에서 헤드폰을 착용하고 화자가 말했을 것이라고 생각되는 단어를 받아쓰도록 하였다. 산출된 말명료도 점수에 따라서, 화자를 말명료도 수준에 따라서 고 집단(n=44), 중 집단(n=33), 저 집단(n=23)으로 구분하였다. 결과: 혼합 이원분산분석을 실시한 결과, 말명료도 수준과 조음오류 유형의 주효과가 유의하였으며, 말명료도와 조음오류 유형간의 상호작용 효과도 유의하였다. 말명료도가 높은 집단일수록 조음오류가 유의하게 적게 나타났으며, 생략, 대치, 쉼의 조음오류 유형에서 말명료도 집단 간의 차이가 유의하게 나타났다. 단계적 중다회귀분석 결과, 마비말장애인의 조음오류 유형 중에서, 생략, 대치, 비구어적 소리만이 말명료도를 유의하게 예측할 수 있었다. 논의 및 결론: 말명료도 수준에 따라서 마비말장애인의 조음오류 유형의 프로파일에 차이가 있었으며, 생략, 대치, 비구어적 소리만이 마비말장애인의 말명료도를 유의하게 예측할 수 있는 변수로 나타났다. 이러한 결과는 마비말장애인의 말명료도 수준과 조음오류 유형을 평가, 치료, 음성 인식 프로그램 개발 시에 고려해야 한다는 것을 시사한다.

영어초록

Background & Objectives: The purpose of the current study was to investigate the articulation error patterns according to the level of speech intelligibility and to examine which articulation error patterns significantly predicted speech intelligibility in adults with dysarthria. Methods: One hundred speakers with dysarthria and 25 native listeners participated in the present study. Based on the levels of speech intelligibility, speakers were classified into three groups: high- intelligibility speakers (n=44), mid-intelligibility speakers (n=33), and low-intelligibility speakers (n=23). APAC words were used as stimuli for obtaining dysarthric speech. Articulation error patterns were classified into eight categories as follows: omission, substitution, distortion, addition of consonants, prolongation, repetition of the phonemes and syllables, intra-word pause, and non-speech sounds. Frequencies of articulation error patterns were examined by speech-language pathologists. Speech intelligibility was judged by 25 native listeners using a word transcription task. Results: Results of the two-way ANOVA (groups x articulation error patterns) revealed that the two main effects were statistically significant, indicating that high-intelligibility speakers showed fewer errors than the rest of the groups, and that substitution had a significantly higher frequency for error than the rest of the patterns. A two-way interaction was statistically significant due to the fact that the group differences emerged only in three articulation error patterns (substitution, omission, and intra-word pause). A stepwise multiple regressions analysis showed that omission, substitution, and nonspeech sounds were significant predictors of speech intelligibility in dysarthria. Discussion & Conclusion: The current study suggested that the articulation error pattern varied depending on the level of speech intelligibility and may differentially contribute to speech intelligibility in persons with dysarthria. The clinical implications of the results are discussed in terms of evaluating dysarthric speech and developing a speech recognition device for dysarthric speakers.
