초록

배경 및 목적: 본 연구에서는 뇌손상 환자에서 『한국판 프렌차이 실어증 선별검사(Korean version of Frenchay Aphasia Screening Test: K-FAST, 이하 K-FAST)』의 신뢰도와 타당도를 검정하였다. 방법: 신뢰도와 타당도 분석을 위하여 50명의 실어증 환자와 42명의 비실어증 뇌손상 환자의 자료를 이용하였다. Chronbach’s ɑ를 통해 문항내적일관성 신뢰도를 측정하였고, Kendall’s coefficient of concordance를 산출하여 검사자간 신뢰도 및 재검사 신뢰도를
검정하였다. 총 검사와 하위 검사 간 상관관계를 분석하여 구인타당도를 측정하였고, 『한국판 파라다이스 웨스턴 실어증 검사(Paradise․Korean version Western Aphasia Battery: P․ K-WAB, 이하 P․K-WAB)』과의 상관관계를 통하여 공인타당도를 검정하였다. ROC 곡선을 분석하고 정상인 평균의 -1 표준편차 값을 적용한 절단점을 이용하여 민감도와 특이도를 구하였다. 결과: 문항내적일관성 신뢰도(총 검사 .88, 간략형 검사 .81), 검사자간 신뢰도(총 검사 .92, 간략형 검사 .94), 재검사 신뢰도(총 검사 .979～ .990, 간략형 검사 .979～ .998)에서 높거나 매우 높은 신뢰도가 입증되었다. 구인타당도(총 검사와 하위 검사 간 .820～ .973, 간략형 검사와 하위 검사 간 .737～ .910)와 공인타당도(총 검사와 실어증지수 간 .904, 간략형 검사와 실어증지수 간 .910) 검정 결과 우수한 타당도가 입증되었다. ROC 곡선 분석 결과에서는 진단유용성을 입증하지 못 하였지만, 절단점을 적용한 민감도와 특이도 산출 결과 특이도는 낮지만 민감도는 높은 검사로 평가되었다(총 검사의 민감도 90.0%, 특이도 31.0%, 간략형 검사의 민감도 88.0%, 특이도 28.6%). 논의 및 결론: K-FAST는 신뢰도, 타당도, 민감도가 높은 검사도구라고 할 수 있다. 그러나 뇌손상 환자들의 특성은 검사의 특이도에 부정적인 영향을 줄 수 있으므로, 실어증 뿐 아니라 기타 인지 의사소통장애 여부를 선별해주는 도구로 K-FAST를 활용할 것을 제안한다.

영어초록

Background & Objectives: Early identification, diagnosis, and treatment of aphasia are
important steps in maximizing rehabilitation gains. A routine screening test is an invaluable tool
in the identification and appropriate referral of patients with potential communication
problems. Nevertheless, there is nostandardized aphasia screening test available in Korea. The
purpose of this study was to test the reliability and validity of the Korean version of the Frenchay
Aphasia Screening Test (K-FAST). Methods: We used test data from 50 aphasic patients and 42
non-aphasic, brain-damaged patients. An internal consistency and intra-/inter-rater
reliabilities of the K-FAST were analyzed by the Cronbach’s ɑ value and Kendall's coefficient
of concordance, respectively. A constructive validity through correlations between the total and
subtest scores and a concurrent validity through correlations between the scores of the K-FAST
and Paradise․Korean version Western Aphasia Battery(P․K-WAB were measured. The
diagnostic accuracy of the K-FAST in brain-damaged patients was measured by the area under
the ROC curve, while the sensitivity and specificity were evaluated using the cut-off value of
the normal data. Results: Results obtained in the present study were as follows: Indices of the
internal consistency and intra-/inter-rater reliabilities of the K-FAST showed good reliabilities.
There were significantly strong correlations between the total and subtest scores of the K-FAST,
and between the scores of P․K-WAB and K-FAST. For detecting aphasia among braindamaged
patients, the sensitivity was high while the specificity was low. Discussion &
Conclusion: Our results show that K-FAST could be a reliable and valid tool for screening
Korean Aphasics. However, the specificity of the K-FAST appeared to be adversely affected by
the characteristics of the brain-damaged patients.
