회상요법을 적용한 집단미술치료가 만성질환 노인의 심리적 안녕감과 무망감 우울증에 미치는 효과 (Effects of Group Art Therapy using Reminiscence Therapy on the Psychological Well-Being and Hopelessness Depression of the Elderly with Chronic Illness)

초록

본 연구는 회상요법을 적용한 집단미술치료 프로그램이 만성질환 노인의 심리적 안녕감과 무망감 우울증에 미치는 효과를 알아보는데 그 목적이 있다. 연구 대상은 충북 J시 B요양원 입소자 중 75세 이상의 만성질환 노인으로 자발적 프로그램 참여를 원하는 실험집단 6명과 프로그램에 참여하지 않는 통제집단 6명으로 총 12명으로 2009년 4월 21일부터 7월 21일까지 주 1회 90분 이상 총 15회에 걸쳐 진행되었다. 프로그램 효과를 검증하기 위해 심리적 안녕감 척도 검사와 무망감 우울증 척도 검사를 사전·사후·추후에 걸쳐 실시하였고 질문지 검사의 한계를 보완하기 위하여 자유만다라 그림검사를 실시하였다. 실험·통제집단의 동질성 검증은 맨 휘트니 U검증을 실시하였고, 효과성 검증은 윌콕슨 부호 순위 검증을 하였다. 연구결과를 바탕으로 얻은 결론은 다음과 같다. 첫째, 회상요법을 적용한 집단미술치료는 만성질환 노인의 심리적 안녕감에 긍정적인 효과가 있는 것으로 나타났다. 둘째, 회상요법을 적용한 집단미술치료는 만성질환 노인의 무망감 우울증에서 통계적으로 유의미한 차이가 나타나지 않았으나, 추후점수에서 인지적 증상과 자존감 저하가 상승하는 것으로 나타났다. 셋째, 회상요법을 적용한 집단미술치료과정에서 감정, 정서, 갈등의 시각적 언어적 표현이 증가됨에 따라 개인의 창조성과 자발성이 향상되었다.

영어초록

This study examined if a group art therapy program using reminiscence therapy could cause any effects on the psychological well-being and hopelessness depression of the elderly with chronic illness. The participants of this study were twelve inmates at a nursing home located in city J, Chungchong- buk-do, all of them over 75 years-old with chronic illness. They were divided into two groups of six: the experimental group which received the group art therapy program using reminiscence therapy, and the control group which did not receive it. The program lasted from April 21st, 2009 to July, 21st 2009. The experimental received a total of 15 sessions, each session 90 minutes long. Measurement of the inmates' psychological well-being and hopelessness depression was conducted at a pre, post and follow-up scale test, and the results were statistically analyzed. In addition, Mandala picture test was conducted in a pre, post and follow up design to complement the limit of paper-based scale tests, but it was excluded from the study as two of the inmates were later hospitalized. Mann-Whitney U test was computed to check the homogeneity of the control and experimental group. Wilcoxon singed rank test was computed for the statistical analysis of this study. The findings of this study are as follows. First, the treatment program caused positive effects on the psychological well-being of the nursing home inmates with chronic illness. Second, the effects of the treatment program on the hopelessness depression of the nursing home inmates was not statistically significant, but later an increase in the further scores of their preception problems and self-esteem was found. Third, the treatment program fostered creativity and initiatives in the inmates as they came to express their emotions, sentiments, and conflicts frequently in linguistically visual words along the therapy sessions.
