초록

목 적：Synthetic diffusion은 다른 두 개의 b-value로 검사를 하는 경우 중간의 어떤 b-value로도 영상을 만들어 낼 수 있는 장점이 있다. Respiration gating을 이용한 liver DWI에서 b-value 50, 400, 800으로 검사하는 경우와 b-value 50, 1000으로 검사하여 후자에서 synthetic DWI로 b-value 400, 800으로 후 처리하여 얻은 영상을 실제로 검사한 영상의 차이와 스캔 타임 감소에 있어서 환자에게 얼마나 유용한지 알아보고자 한다.
대상 및 방법：2018년 11월부터 2019년 1월까지 본원에서 liver MRI를 검사한 환자 18명(남: 14명, 여: 4명, 평균나이: 46세)을 대상으로 하였고 일반적인 liver DWI과 synthetic DWI의 각 b-value 400, 800에서 간실질, 담낭에서 ROI를 측정하여 SNR, ADC값(mm2/sec)을 비교하였고, SPSS의 paired sample t-test, Pearson’s correlation을 이용하여 정량적 분석을 하였다. 장비는 3.0T Architect(GE healthcare)으로 body array coil과 spine coil을 사용하였으며 일반적 DWI과 synthetic DWI의 영상매개를 모두 같게 하였다.
결 과：Liver DWI와 synthetic DWI의 b-value 값에 대한 SNR을 비교한 결과 b-value 400에서는 간실질, 담낭 모두 통계적 유의성을 확인하였다.(p<0.05) b-value 800에서도 간실질, 담낭에서 유의함을 확인하였으며,(p<0.05) ADC값 (mm2/sec)은 간실질에서 1.03×10-3 담낭에서 2.58×10-3 synthetic DWI에서 간실질 0.94×10-3, 담낭 2.52×10-3로 상관관계 분석에서도 DWI과 synthetic DWI의 ADC값(mm2/sec)이 밀접한 관련이 있음을 확인하였다.
결 론：대부분의 liver MRI에서 HCC 또는 hemangioma같은 병변을 진단하기 위해 DWI multi b-value를 검사하는 경우가 대부분이지만, 본 논문에서는 DWI b-value를 두 가지(50, 1000)를 사용하여 synthetic DWI으로 원하는 b-value를 후처리해도 SNR과 ADC에서 큰 차이가 없음을 확인하였다. 시간상으로도 respiration gating DWI multi b-value(50, 400, 800)는 평균 6분 21초 걸렸으며, synthetic DWI은 3분 9초로 약 절반가량 줄어들었다. Respiration gating 사용하는 liver MRI는 환자의 호흡이 불안정하거나 숨 멈춤이 있을 경우 장비적으로도 멈춤이 있을 수 있고, 처음부터 다시 검사하는 경우나 환자가 호흡으로 인해 어려움을 겪을 경우가 있는데 synthetic DWI은 시간상으로도 환자 측면에서 유용한 검사 로 이용될 수 있다.

영어초록

Purpose：If two b-values are examined, the synthetic diffusion has the advantage of producing an image with any middle b-value. By examining b-values of 50, 400, 800 in the liver DWI with respiration gating and b-value 50, 1000 by synthetic DWI, images obtained by post-processing 400, 800 values of synthetic DWI, the usefulness to patients in terms of differences in the values actually examined and time reduction will be determined.
Materials and Methods：From November 2018 to January 2019, 18 patients (14 males and 4 females, mean age: 46 years) who underwent liver MRI examination at this our hospital were subject of this study. The hepatic parenchyma from each b-value of general liver DWI and synthetic DWI, and the ROI were measured in GB, and the SNR and ADC(mm2/sec) values were compared. By using the paired sample t-test and Pearson's correlation of SPSS, the quantitative analysis was done. The scanner that was used was a 3.0T Architect(GE healthcare) using a body array coil and both the DWI and synthetic DWI image parameters were the same.
Results：As a result of comparing the SNR for the b-value of general liver DWI and synthetic DWI, both liver and GB showed statistical significance in the b-400.(p<0.05) In b-800, it was also significant at the liver and GB(p<0.05). The ADC(mm2/sec) values were 1.03×10-3 in the liver and 2.58×10-3 at GB, the synthetic values were 0.94×10-3 at in the liver and 2.52×10-3 in the GB. The correlation analysis also confirmed that the general DWI and synthetic DWI were closely related.
Conclusion：In most liver MRI, DWI multi b-value is often used to diagnose lesions such as HCC or hemangioma, however, in this paper, it was confirmed that there was no significant difference between SNR and ADC even when the desired b-value is post-processed by synthetic using two DWI b-values(50, 1000). In terms of time also, the respiration gating DWI multi b-value(50, 400, 800) took an average of 6 minutes and 21 seconds and Synthetic DWI was 3 minutes and 9 seconds, which was reduced by about half. During liver MRI using respiration gating, there may be difficult cases such as if the breathing of patient is unstable or stops breathing, pauses during the scan, there may be cases where the patient is examined again from the beginning, or the patient often suffers from breathing difficulties. However, the use synthetic DWI can be used as advantageous for patients in terms of time.
