초록

목 적 : 뇌 관류검사에서 일곱번의 exponential delay spacing PLD(post labeling delays)로 brain perfusion영상을 획득하여 재구성한 corrected rCBF(cerebral blood flow)map과 single PLD로 획득, 재구성한 rCBFmap을 비교하여 그 진단적 유용성을 평가하고자 한다.
대상 및 방법 : 2016년 6월부터 10월까지 뇌 혈관성 질환 의심 및 확진 환자 30명(환자의 평균 연령은 53.1세이고 남자12명 여자18명)을 대상으로 연구하였다. MR system은 Discovery 750W(General electronics. USA), 32channel head coil을 사용하였다. single delay ASL은 512 sampling points on 8 spirals, TR:4466ms, TE:10ms, NEX:2, post label delay:1525ms 의 권고 parameter, 7delays eASL은 640 sampling points on 4 spirals, TR:5868ms, TE:10ms, NEX:1의 권고 parameter 를 제외한 나머지 parameter 인 slice thickness: 6mm, number of slice:26, labeling plane: acquisition volume하방 20mm 로 동일하게 하고, post labeling delay를 각각 1회, exponential spacing 7 delays(label duration 1000ms~3500ms)로 하여 spirals 3D fast spin echo imaging sequence로 데이터를 획득, rCBFmap과 corrected rCBFmap을 비교하였다. 영상평가는 정량적 평가와 정성적 평가로 나누어 실시하였다. 정량적 평가는 각각의 rCBFmap에서 상대적 정상 flow가 유입된다고 판단되는 cerebellum의 CBF-value를 100%기준으로 하여 lesion이 확실한 단면, 또는 중간 번째 단면의 rCBFmap에 frontal lobe 4, parietal lobe 2, occipital lobe 2, 총 8구역에 동일한 크기의 ROI를 통해 측정한 CBF-value와의 대비(%)로 수치화 하여 평가하였고, 정성적 평가는 영상의학과 두경부 신경계 판독의 1명과 MRI실에 근무경력 3년 이상인 방사선사 2명 을 대상으로 리커트(Likert scale) 5점 척도로 평가하였다. 정량적, 정성적 평가 모두 SPSS 23.0(SPSS INC. Chicago. IL. USA)을 이용하여 p<0.05일 때 통계적으로 유의한 것으로 판단하였다. 측정, 평가자간 신뢰도는 반복 측정치를 이용한 급 내 상관계수인 ICC(intraclass correlation coefficient)를 구하여 0.75(p<0.05) 이상 나올 경우 신뢰성이 높은 것으로 간주하였다.
결 과 : 정량적 평가에서는 각 측정 부위에 대한 백분위 상대치의 평균값에서 측정 region A에서만 7delays eASL기법이 0.61낮은 수치를 보였을 뿐 B~H에 이르는 전 구간에서 각각 4.32, 7.69, 18.33, 11.46, 26.26, 16.47, 19.9만큼의 상대치 평균 차이가 높은 값을 보였고 모든 측정부위의 p-value는 p<0.05로 통계적으로 유의한 차이를 보였다. 측정자간 신뢰도는 0.805~0.869(p<0.05)로 나타났다. 정성적 평가에서는 리커트 5점 척도 결과 single delay ASL, 7delays eASL 각 각 혈 류신호 균일성에서 2.75±0.94, 4.19±0.78 , 백질 회백질 구분에서 3.64±0.57, 4.08±0.81, 지연혈류 표현에서 2.25± 0.95, 4.16±0.83, 관류대조도 표현에서 3.75±0.50, 4.75±0.50의 결과를 보였다. 비교결과 7delays eASL의 평균 점수가 모두 높았으나 백질 회백질 구분 항목에서 p>0.05(p=0.130)으로 통계적 유의한 차이는 보이지 않았고 나머지 항목에서는 p<0.05(p=0.006, 0.007, 0.004)로 통계적으로 유의한 차이를 보였다. 평가자간 신뢰도는 0.774~0.854(p<0.05)로 통계적 으로 높은 신뢰도를 나타냈다.
결 론 : 7 delays eASL을 이용하여 획득한 뇌관류(brain perfusion)영상은 single delay ASL의 영상과 비교했을 때 진단적으로 유효하다고 판단할 수 있다. 비교적 긴 시간 동안 혈류의 정보를 획득함으로 검사과정에서 발생 가능한 기술적 오류를 줄이고, 상대적으로 놓칠 수 있는 지연혈류의 확인이 가능한 7delays eASL 기법은 내인성(endogenous) 관류 획득 방법으로, 진단적으로 보다 개선된 뇌 관류영상을 얻는데 유용한 방법이라 판단된다.

영어초록

Purpose : It is to evaluate diagnostic usefulness by comparing corrected rCBF (cerebral blood flow) map which has been reconstituted with brain perfusion images with seven times of exponential delay spacing PLD (post labeling delays) and rCBFmap acquired in single PLD. Materials and Methods : The subjects of this study were 30 cerebrovascular disease suspected or diagnosed patients (mean age was 53.1, 12 males and 12 females). The study was performed from June to October in 2016. The MR system used Discovery 750W (General electronics, USA) and 32channel head coil. Each parameter was defined as 512 sampling points on 8 spirals, TR: 4466 ms, TE: 10 ms, NEX: 2, post label delay: 1525 ms (single ASL), 640 sampling points on 4 spirals, TR: 5868 ms, : 1, the post label delay: 1000 ~ 3500 ms (7delay eASL) Slice thickness: 6 mm, number of slice: 26, labeling distance 20 mm, And rCBFmap and corrected rCBFmap were compared.. Image evaluations were made quantitatively and qualitatively. In quantitative evaluation, in each rCBFmap quantification by contrast with cBF-value (%) measured through ROI of the same size in total 8 zones such as 4 frontal lobes, 2 parietal lobes and 2 occipital lobes on the section with conspicuous lesion or on the middle section of rCBFmap based of CBF-value of cerebellum which is judged to have relatively normal inflow (100%). In qualitative evaluation, a radiologist specialized in derencephalus nervous system and 2 radiologic technicians who have been working in MRI chamber for more than three years were asked to answer the survey questions in Likert 5 point scale. Both of quantitative and qualitative evaluations were verified using SPSS 23.0 (SPSS INC. Chicago. IL. USA) and were considered statistically significant when p<0.05. For measurement and inter-rater reliability, ICC (Intraclass Correlation Coefficient) was calculated using repetitive measurements. It was considered highly reliable when it was over 0.75 (p<0.05)
Results : In quantitative evaluation, in average of percentile counterparts in each measurement part, 7delays eASL method showed lower score by 0.61 in region A only, while it showed higher score by 4.32, 7.69, 18.33, 11.46, 26.26, 16.47 and 19.9 than percentile counterparts in whole slots from B to H. p-value in all measured area was statistically significant as p<0.05. Inter-rater reliability was 0.805~0.869(p<0.05). In qualitative evaluation through Likert 5 point scale, single delay ASL and 7delays eASL showed 2.75±0.94 and 4.19±0.78 in blood flow signal uniformity, 3.64±0.57 and 4.08±0.81 in distinction of white and gray matter, 2.25±0.95 and 4.16±0.83 in delayed blood flow expression and 3.75±0.50 and 4.75±0.50 in perfusion contrast expression. Although 7delays eASL showed higher average points in all items, there was no statistically significant difference in distinction of with and gray matter as p>0.05(p=0.130). In all other items, there was statistically significant difference as p<0.05(p=0.006, 0.007, 0.004). Inter-rater reliability was 0.774~0.854(p<0.05), which confirms that it is highly reliable.
Conclusion : Brain perfusion images acquired by 7 delays eASL are supposed to be more diagnostically effective than images acquired by single delay ASL. As it acquires blood flow information for relatively longer time, it reduces technical errors during the test process and can identify delayed blood flow which can be missed in single delay ASL. It can be said that 7delays eASL method is an endogenous perfusion acquisition method which is very useful to get diagnostically improved brain perfusion image
