만성폐질환의 어원 그대로 해석하면 1년이상부터 평생까지 오랜 시간에 걸쳐 병세가 지속되는 기도 및 폐에 생겨 막히거나 흐름을 방해하는 질환이다. 구체적으로 만성 폐질환은 유해한 입자(담배가 대표적)나 가스의 흡입에 의해 발생하는 폐의 비정상적인 염증반응과 이에 동반되어 완전히 가역적이지 않으며 점차 진행하는 기류제한을 특징으로 하는 호흡기 질환이다.[48]

-위험성-
담배를 피우면 폐암에 걸릴 수 있다는 사실은 누구나 알고 있지만 만성폐쇄폐질환으로 사망할 수 있다는 사실은 잘 알려져 있지 않다. 현재 만성폐쇄폐질환의 사망 원인은 전세계적으로 4위를 차지하고 있다. 하지만 세계보건기구(World Health Organization: WHO)는 2020년쯤에는 사망원인 3위, 장애원인 5위로 부상할 것으로 예상하고 있다.
우리나라의 경우, 지난 2003년 대한 결핵 및 호흡기학회가 전국 성인남녀 9243명을 대상으로 한 ‘만성폐쇄폐질환 전국 실태조사’에 따르면, 국내 45세 이상 성인의 17.2% (남성 25.8%, 여성 9.6%)의 유병률을 가진 것으로 나타났다. 2008년 국민건강영양조사에서도 40세 이상의 성인 중 13.4% (남성 19.4%, 여성 7.9%) 가 만성폐쇄성폐질환을 가지고 있는 것으로 조사되었다.
문제는 이런 높은 유병률에도 불구하고 20년 이상 담배를 피우고 호흡곤란 증상까지 있는 잠재환자의 92%가 병원진료조차 받지 않을 정도로 방치되고 있는 실정이다. 만성폐쇄폐질환이 무서운 것은 폐 기능이 50% 이상 손실되기 전까지는 증상이 나타나지 않아 초기에 진단이 어렵다는 것이다.[48]

-연구의 중요성-
일단 증상이 나타나면 급속히 악화되고 어떠한 약물치료도 폐기능을 호전 시킬 수 없으며 중증이 되면 24시간 지속적인 ‘산소요법’만이 생명을 연장시킬 수 있을 뿐이다. 한번 손상된 폐 기능은 다시는 회복되지 않기 때문인데 따라서 조기진단과 병의 악화를 막는 치료가 매우 중요하다.[48]
