지난 2010년 9월, 한 남성이 5.18 구속 부상자회로 편지를 남기고 자살을 했다. 그는 편지에 “꿈에 항상 군인들이 나타나 살 수가 없습니다. 고문 후유증으로 살 수가 없습니다”라는 글을 남겼다. 5.18 광주 민주화 운동 유공자였던 그는 30년간 우울증과 불면증으로 고통 받다가 스스로 목숨을 끊은 것인데, 이는 심각한 ‘외상 후 스트레스 장애(PTSD: posttraumatic stress disorder)’로 인한 것이었다. 2011년 12월까지 5.18 부상 후유증으로 숨진 사람 380명 중 42명이 자살로 세상을 떠났다. 2007년에 5.18 부상자와 구속자, 유족 등 유공자 113명을 대상으로 실태를 조사한 결과 응답자의 25.6%가 가벼운 외상 후 스트레스 장애 증상을 보였고, 16.8%는 외상 후 스트레스 장애 진단을 받았다. 이런 상황을 개선하기 위해 2012년 6월 광주광역시가 ‘5.18 트라우마 센터’를 개원할 예정이며, 센터에서는 피해자에 대한 전문적 치료 프로그램을 개발하고 서비스를 제공한다고 한다.
위의 사례에서 등장하는 외상 후 스트레스 장애(이하 PTSD)는 충격적인 사건을 경험(직접 혹은 목격)하고 난 후에 불안상태가 지속되는 경우를 말한다. 여기서 외상(trauma)이라 함은 죽음이나 심각한 신체적 손상을 초래하는 매우 충격적인 사건을 말한다. 이러한 충격적 사건을 경험하거나 목격한 후에는 다음과 같은 세 가지 유형의 심리적 증상이 나타난다. 첫째, 외상적 사건을 지속적으로 재경험하는 증상들이 나타난다. 둘째, 트라우마와 관련된 자극을 회피하거나 정서적으로 무감각해진다. 셋째, 예민한 각성상태가 지속된다. 이러한 증상을 나타내는 PTSD의 원인과 특성을 설명하는 입장들은 다양하게 존재하지만, 이 글에서는 인지적 입장에 초점을 두어 다루기로 한다. 여기에서 주로 다룰 것은 PTSD가 지속되는 사람들의 인지과정의 특성이다.
PTSD와 인지 심리학의 연관성에 대해 살펴보고자 하는 데에는 몇 가지 의의가 있다. 첫째, 인지 심리학 분야에서 실험들을 통해 확인된 원리가 다른 분야의 심리학에 어떠한 방식으로 적용될 수 있는지 알아보고 그 이해의 폭을 넓힐 수 있다.
