Ⅰ. 서 론
Kanner(1943)가 처음 자폐증을 언급한 이전부터 자폐증과 비슷한 증상을 지칭하는 용어는 수없이 많았다. Laufer와 Gair(1969)는 이러한 용어가 20가지 이상 있지만, 용어간에 개념을 명확하게 구분하지는 않았음을 지적하였다. Kanner가 11명의 아동을 임상 실험한 결과가 보고된 이후에도 자폐증의 개념에 대하여 많은 혼란을 겪어 온 것이 사실이다. 그러나 1980년 미국 정신의학회에서 수많은 연구결과와 임상 경험을 바탕으로 자폐증을 전반적 발달장애로 공식적인 명칭을 부여하면서 부터 이전까지 정신병적 상태로 이해되었던 자폐증이 발달상의 심각한 장애로 재인식되는 획기적인 개념상의 변천을 가져왔다. 또한 서로 상관관계에 있는 군집 행동들을 개개 행동군으로 범주화하여 정서·행동장애에 대한 양적 차원 분류체계를 제시한 Quay(1986)는 자폐증을 병리적으로 가장 심한 정도의 분류영역인 정신증적 행동내에 포함시켰다.

이와 같은 자폐증 개념에 대한 변화와 함께 자폐적 증상을 가진 아동에 대한 관심이 증폭되었고, 이전보다 더 많은 수의 아동이 자폐 범주에 속하게 되었다. 또한 개념의 변화와 더불어 세계 각국에서 보고되고 있는 자폐아 출현율은 1980년대 이전에 비하여 2배 이상 증가하였는데, 우리나라의 경우 지금까지 자폐아 출현율에 대한 보고가 거의 전무한 상태여서, 출현율의 과다 정도에 따른 적절한 교육중재 및 지원 조치를 위한 기본 계획의 수립이 불가능한 상황이다.

이에 본 연구는 DSM-Ⅳ상에 나타난 분류체계에 따라 자폐 장애와 달리 분류되지 않는 전반적 발달장애를 중증 행동장애로 통합, 개념화하여 일정 지역의 중증 행동장애 출현율을 밝혀보고자 하였다.

Ⅱ. 출현율 관련 선행연구
유아자폐증이 처음 언급된 이래, 그 출현율에 대한 조사연구는 약 20년 후부터 세계 여러 지역에서 이루어지기 시작하였다. 이 연구결과를 표로 정리하면 다음과 같다.
