착용형 남성용 소변 수집기의 사용성 평가 (Usability Test of a Wearable Urine Collecting Device for Men)

초록

노인 와상 환자 및 선천적 혹은 사고로 인한 중증 요실금 환자들의 남성용 착용형 소변 수집기에는 요도 카테터와 콘돔 카테터가 있다. 요도 카테터는 요도를 통하여 방광에 삽입된 카테터로 소변을 수집하며, 콘돔 카테터는 음경을 감싸는 콘돔형태의 소변 수집기로 외부의 소변 백에 연결된다. 두 제품 모두 감염의 염려가 있으며, 콘돔 카테터의 경우에는 음경 피부와 장시간 접촉하면서 피부염증과 괴사를 유발하기도 한다. 본 연구에서는 피부를 감싸지 않고 팬티에 부착할 수 있는 착용형 남성용 소변 수집기의 사용적합성을 시험하였다.
착용형 남성용 소변 수집기의 직경은 남성 음경의 평균 직경인 3cm보다 큰 직경 5cm의 원통형으로 길이는11cm이며 전용 팬티에 부착할 수 있다. 사용성 평가 시험에서는 쪼그려 앉은 자세에서 음경이 수집기에서 이탈할 확률과 하룻밤 수면 중 음경이 수집기에서 이탈할 확률을 조사하였다. 또한 설문 조사를 통하여 착용감 및 피부 부작용 여부를 평가하였다.
소변 수집기의 착용은 팬티의 외측에 부착하는 방법과 내측에 부착하는 방법이 있다. 쪼그려 앉은 자세에서 팬티의 내측에 착용하는 방법이 더 이탈가능성이 낮았으며, 하룻밤 수면후의 이탈 여부 조사에서도 팬티의 외측에착용하는 방법이 약 58% 이탈하여 팬티의 내측에 착용하는 경우의 33%에 비하여 이탈률이 높았다. 실리콘으로제작된 소변 수집기의 착용감은 수면 중 큰 불편을 주지 않는 정도였으며, 4명의 남성 시험 착용자 중 한 명이약간의 가려움이 있었다고 보고하였다.
새로 개발된 남성용 착용형 소변 수집기는 피부 부작용의 가능성은 낮으나, 사용중 이탈 가능성이 있으므로 성인용 기저귀와 겸용으로 사용할 것을 권장한다.

영어초록

Urethral catheters and condom catheters, also called urinary sheaths, are widely used products for collecting urine from men with severe incontinence. The urethral catheter collects urine with a catheter inserted into the bladder through the urethra, and the condom catheter collects urine from a condom-type urine collector surrounding the penis. The collected urine is then transferred and stored to an external urine bag through a urine tube. Both products may cause infection, and in the case of condom catheters, prolonged contact with the skin of the penis may cause skin inflammation and necrosis. In this study, the suitability of a new wearable male urine collector that can be attached to panties without contacting the skin was tested for usability.
The new wearable men's urine collector has a cylindrical shape with a diameter of 5 cm, a length of 11 cm, and can be attached to a specially designed underwear. As the diameter of this urine collector was larger than the average diameter of male penis of 3 cm, the probability of the penis moving out of the collector, dislocation probability, when the user was in a squatting position as well as the probability after the collector had been worn overnight were investigated.
In addition, wear comfortability and side effects were evaluated through a survey.
Two methods of urine collector attachment onto the underwear were applied: internal and external attachment, where the urine collector was attached inside and outside of the underwear, respectively. In a squatting position, the dislocation probability was lower with the internal attachment. The dislocation probability after one-night sleep was also lower with the internal attachment, 33 %, compared to that with the external attachment, 58%. Wearing of the urine collector made of silicone did not cause any major discomfort during sleep, nut one test wearer reported slight itching.
The newly developed men's wearable urine collector has low possibility of skin side effects, but it is recommended to be used in combination with urine absorbent pads because there is a possibility that the urine collector could be dislocated from the normal position during use.
