간헐 외사시에 대한 양안 외직근 후전술 후 발생한 속발 내사시 치료를위한 외직근 전진술의 장기 효과 (The Long-Term Outcome of Lateral Rectus Advancement in Patients with Consecutive Esotropia Following Bilateral Lateral Rectus Recession for Intermittent Exotropia)

초록

목적: 간헐 외사시에 대한 양안 외직근 후전술 후 발생한 속발 내사시의 치료로서 외직근 전진술의 24개월 이상 장기 결과를 알아보고자 하였다.
대상과 방법: 양안 외직근 후전술 후 발생한 속발 내사시로 외직근 전진술을 시행하고 24개월 이상 경과 관찰이 가능했던 25명 환자의의무기록을 후향적으로 분석하였다. 속발 내편위각이 처음 외편위각과 비슷하거나 보다 커서 양안의 외직근 전진술을 시행한 16명의환자를 BLR군으로, 속발 내편위각이 처음 외편위각보다 작아 단안 수술을 시행한 9명의 환자를 ULR군으로 분류하여 수술 성공률을비교하였고, 각 군에서 수술의 효과를 알아보기 위해 총 편위각 교정량을 외직근 전진술의 수술량으로 나눈 수술효과-수술량 관계를분석하였다. 운동 성공은 정위에서 10 prism diopters (PD) 미만 외편위로, 감각 성공은 티트무스 입체시 검사상 100 arc sec 이내의경우로 정의하였다.
결과: 외직근 전진술 시행 후 총 25명 환자 중 18명(72.0%)에서 양호한 운동 기능을 보였고, 티트무스 입체시 검사가 가능하였던 총22명의 환자 중 17명(77.3%)에서 양호한 감각 기능을 보였다. 최종 사시각을 기준으로 한 성공률은 ULR군이 BLR군보다 통계적으로유의하게 좋았다(p=0.025). 수술효과-수술량 관계는 전체 환자에서 최종 관찰 시 3.7 PD/mm였으며, ULR군에서 BLR군에 비해 유의하게 단위 수술량당 교정효과가 큰 결과를 보였다(p=0.043).
결론: 외직근 전진술은 속발 내사시의 치료로서 전반적으로 양호한 장기 수술 결과를 보였지만, BLR군 중 속발 내사시와 처음 외사시의 편위각이 비슷한 환자에서는 수술효과-수술량 관계를 고려한 수술량의 조정이 필요할 것으로 생각한다.

영어초록

Purpose: To investigate the long-term outcome of lateral rectus (LR) advancement for consecutive esotropia following bilateralLR recession for intermittent exotropia.
Methods: Medical records of 25 patients who underwent LR advancement for consecutive esotropia after bilateral LR recessionand who were followed up for more than 24 months postoperatively were reviewed. Patients were divided into two groups: bilaterallateral rectus recession (BLR) group included 16 patients with consecutive esotropia greater than or the same magnitudeas the initial exotropia and who underwent bilateral LR advancement; Unilateral lateral rectus recession (ULR) group includednine patients with consecutive esotropia less than the initial exotropia and who underwent unilateral LR advancement. Main outcomemeasurements were motor and sensory outcomes and the dose-effect relationship calculated from observed overall andgroup changes in the angle of deviation per millimeter. Motor success was defined as alignment from orthotropia to exodeviationless than 10 PD at distance. Sensory outcome was described by comparing the Titmus stereoacuity test before and after LRadvancement. Sensory successvwas defined at 100 seconds of arc.
Results: Eighteen patients (72.0%) showed satisfactory long-term motor and sensory outcomes. Seventeen (77.3%) of 22 patientsshowed favorable stereopsis of 100 seconds of arc or more at final observation. The long-term motor success rate of the ULR groupwas better than that of the BLR group (p = 0.025). The average observed change in the angle of deviation was 3.6 PD/mm at the finalvisit in all patients. A greater dose-effect relationship was observed in the ULR group than in the BLR group at the final visit (p = 0.043).
Conclusions: LR advancement showed favorable motor and sensory outcomes in the majority of patients. The surgical outcomewas not favorable in patients in the BLR group with consecutive esotropia of the same magnitude as the initial exotropia. Theseresults require further investigation for verification.
