선천성면역결핍질환아 간호과정에서의 시간의 경과에 따라 가족이 경험하는 어려움에 관한 연구 (A Study on the Difficulties of Family Members over Time with the Nursing Process of Children with Acquired Immune Deficiency Syndrome)

초록

본 연구의 목적은 선천성면역결핍질환아 간호과정에서의 시간의 경과에 따라 가족구성원들이 당면하게 되는 어려움과 그 차이가 무엇인지를 파악함으로써 환아의 가족구성원들을 위한 임상사회사업 실천방안을 모색하고자 하는 것이다. 본 연구는 생후 2개월부터 7년간 아들의 불치병을 치료하는 과정에서 발생한 딸의 문제와 부부갈등으로 인해 가족상담을 의뢰하여 총 13회기 동안 진행된 가족치료 사례를 Miles와 Huberman(1994)의 질적 분석방법을 활용하여 가족들의 어려움을 분석한 질적 사례연구이다.
본 연구는 분석결과 최종적으로 발견된 주제들을 통해 시간의 경과에 따른 가족구성원들의 어려움과 그 차이점을 살펴보았다. 모의 경우 불안정한 생활로 인한 삶의 질 저하, 기대에 미치지 못하는 남편에 대한 원망감, 다른 자녀와의 정서적 교류 부족과 스트레스, 아들을 보내려는 마음과 망설임, 부의 경우 술·담배·성적 욕구 참아내기, 압박감과 구속감으로 인한 피곤한 일상, 우회적 감정 투사로 스트레스 해소하기, 피곤한 일상에서 탈피하고 싶은 유혹, 아들의 생명 운명에 맡기기, 다른 자녀의 경우 이분화된 부모와의 관계 유지, 부부갈등 상황에서의 희생양, 허술한 둥지에서의 외로움, 외현적 문제행동인 것으로 분석되었다. 이러한 연구결과는 선천성면역결핍질환아 가족들이 직면하는 복합적인 문제를 예방하고 해결하기 위한 기초 자료 제시 및 실질적인 임상실천 방안을 제시하였다고 사료된다.

영어초록

The purpose of this study was to seek clinical social work intervention for the families of children with acquired immune deficiency syndrome through understanding the differences which family members experience in the nursing process of children with acquired immune deficiency syndrome. The study analyzed a family therapy case in which the parents had requested family counseling due to their marital conflict and their daughter's problems, which had arisen in the process of treating their seven-year-old son's incurable disease since he was from 2 months old. The data consisted of recorded transcripts and notes taken during 13 family therapy sessions, which included individual, couple, and family counseling sessions. The study analyzed the mother's experiences, father's experiences, and the sibling's experiences by utilizing the matrix & network developed by Miles and Huberman (1994).
The findings of the study are as follows. First, the results of this study revealed the mother's experiences to be as follows: resentment toward a disappointing husband, the lack of emotional transactions with the other child and stress, thoughts of leaving her son and hesitancy, and degradation in the quality of life due to an unstable life. Second, the study found the father's experiences to be as follows: relief of stress by detouring emotional projection, leaving the son's life up to destiny; suppressing the desire for drinking, smoking and sex; a tiring daily life due to pressure and restrictions; and the temptation to escape from this tiring daily life. Finally, the findings of the study showed the sibling's experiences to be as follows: keeping a divided relationship between her parents, becoming a scapegoat in the situation of parental conflict, anxiety in the situation of parental conflict, loneliness in the lax nest, and the externalization of conscious and unconscious problem behaviors. Based on these findings, the study suggested clinical interventions to prevent and solve the complex problems encountered by the families of children with acquired immune deficiency syndrome.
