안면조직 고정용실의 객관적 임상평가 지표 제시에 관한 연구 (Study on Objective Indicators of Clinical Evaluation of Facial Tissue Fixation)

초록

경제 성장에 따른 소득수준의 증가는 미용 의료기기 시장의 확장으로 이어지고 있으므로, 많은 산업 동향자료를 통해 이를 확인할 수 있다. 미용 목적 의료기기는 의료기기 정의(의료기기법 제2조)에 포함하지 않아, 다른 의료기기에 비해 규제에 대한 정의가 명확하게 이루어지지 않았다. 많은 국내 의료기기 업체들은 동등성 비교를 위해 기 허가된 안면조직 고정용실의 임상 자료를 그대로 반영하여 “주름의 깊이, 주름의 개선”에 집중한 임상을 시행하였고, 이는 실제 안면조직 고정용실이 환자에 적용되는 시술과 많은 부분 상이함이 존재하였다. 본 연구에서는 미국의 미용 의료기기, 특히 안면조직 고정용실에 대한 규제 사항을 확인하고, 시술자와 소비자의 안전성과 유효성을 위해서는 실제 임상시험의 디자인에서 고려해야 할 점을 검토하였다. 다양한 유효성 지표들 중에 최근 많은 의료기기의 안전성을 확보하기 위해 검토하는 CM&S (Computational Modeling and Simulation) 가이드라인과 적용 사례를 확인하였고, 안면조직 고정용실이 생체 내의 물리적인 작용을 하는 점과 생분해성의 소재라는 점을 고려할 때 이러한 기계, 물리적, 생분해 성질 등의 분석은 임상시험을 디자인할 때에도 좋은 참고가 될 수 있을 것이라는 가능성을 확인하였다. 안면조직 고정용실을 비롯한 미용 의료기기의 경우, 이상 사례가 존재하는 시장이지만, 치료에 관한 상해로 인해 시술 전으로 되돌리는 수술이 필요한 경우가 많다. 본 연구에서는 조사된 자료들을 통하여, 안면조직 고정용실의 허가 시 요구되는 안전성과 유효성은 어떤 것인지 살펴보고, CM&S (Computational Modeling and Simulation)의 도입으로 마련할 수 있는 과학적 근거에 대해서 연구해 보고자 한다.

영어초록

The increase in income level due to economic growth is leading to the expansion of the aesthetic medical device market, and this can be confirmed through many industry trend data. Since medical devices for aesthetic purchases are not included in the definition of medical device(Medical device law article 2), the definition of regulations is not clear compared to other medical devices. Many domestic medical device companies have focused on “wrinkle depth and wrinkle improvement” by directly reflecting the clinical data of previously approved facial tissue fixation threads for equivalence comparison, and there were many differences between this and the actual treatment applied to patients with facial tissue fixation threads. In this study, we confirmed the regulations on aesthetic medical devices, especially facial tissue fixation threads, in the United States and reviewed the points to be considered in the design of actual clinical trials for the safety and efficacy of practitioners and consumers. Among the various validity indices, we confirmed the CM&S (Computational Modeling and Simulation) guidelines and application cases that have been recently reviewed to secure the safety of many medical devices, and considered that facial tissue fixation threads perform physical actions in the body and are biodegradable materials, and confirmed the possibility that such analyses of mechanical, physical, and biodegradable properties can be a good reference when designing clinical trials. In the case of aesthetic medical devices including facial tissue fixation threads, there are abnormal cases in the market, but in many cases, surgery to return the device to its pre-procedure state is necessary due to treatment-related injuries. In this study, we will examine what safety and validity are required for approval of facial tissue fixation threads through the surveyed data, and study the scientific basis that can be provided by the introduction of CM&S (Computational Modeling and Simulation).
