보건소 업무 중요도 및 공간 적절성에 관한 연구 -일개 시 보건소를 중심으로- (A Study on the Task Importance and Appropriateness of Space of Community Health Centers -With a Focus on Community Health Centers in a City-)

초록

과거의 보건소의 기능이 전염병 관리나 가족계획사업과 같은 1차 보건사업에서 지역사회 주민을 위한보건으로 점차 확대되고 다각화로 변화됨에 따라 보건소 업무에 따른 보건소 공간의 적절성에 대한 논의가 필요한 시점으로 유기적으로 발전되고 있는 보건소의 업무와 프로그램, 이와 관련된 공간의 적절성에대한 연구의 필요성이 대두되고 있으나, 이에 관한 연구가 미비한 실정이다.
따라서 본 연구에서는 지역적 특성에 따른 보건소사업프로그램이 다양해짐에 따라 현재 보건소 업무중요도와 공간 적절성을 분석함으로서 향후 지역주민의 보건 서비스를 위한 보건소 공간계획에 필요한 기초 자료 및 가이드를 제공하는 것을 연구의 목적으로한다.
연구방법은 설문지법을 활용하였으며, 설문조사는경기도 소재 지역의 보건소 직원 126명을 대상으로설문을 실시하였다. 국내외 정보 D/B 및 논문과 단행본을 포함한 도서⋅출판물 등 문헌조사와 1차 예비조사를 실시하였다. 1차 예비조사는 보건소에 20년 이상근무 경력의 직원 3명을 대상으로 Depth Interview 방식을 활용하여 보건소 각 기능과 공간의 특성을 분류하였으며, 이를 바탕으로 설문지 설계, 조사, 분석의순으로 진행되어졌다.
본 연구의 범위와 내용에는 논문, 관련서적 및 참고문헌의 선행연구를 통해 보건소 업무 중요도와 공간구성의 적절성에 대한 고찰과 설문지를 통한 분석 및보건소 업무공간에 관련한 제언이 포함되어진다.
업무중요도 분석 결과, 업무중요도의 125점(5점) 중평균 96.15점(3.85점)으로, 이 중 결핵관리가 업무중요도가 가장 높았고, 금연사업, 보건교육, 만성질환 순으로 업무중요도가 높은 것으로 나타났으며, 건강검진이업무중요도가 가장 낮은 것으로 타나났다.
공간적절성은 165점(5점) 중 평균 94.55점(2.87점)으로 공간적절성이 낮게 나타났으며, 예방 접종실이 공간적절성이 가장 낮고 다음은 방문보건실, 민원실 순으로 공간적절성이 낮은 것으로 나타났다.
업무의 중요도와 공간 적절성을 현재 보건소에서운영 중인 조직으로 구분하여 분석한 결과, 질병관리와 행정사무를 제외한 의료검사, 건강증진, 지역보건, 감염병 관리는 업무의 중요도와 공간적절성이 역상관관계가 있는 것으로 나타나 업무 중요도에 비해 공간은 협소한 것으로 분석되어졌다.
이러한 결과는 현재 보건소에서 업무 중요도를 고려하지 않고 공간이 기획되어진 것으로, 향후 보건소공간 계획 수립 시 업무 중요도를 고려한 공간 배분과 지역 사회와의 유기적인 연계를 위한 공간 계획의기초자료를 제공하고자 한다.

영어초록

In the past, community health centers used to engage in primary health projects including the management of infectious diseases and family planning.
Today their functions have become extensive and diversified gradually for the health of residents in the community. It is time to initiate discussions about the appropriateness of space in community health centers according to their tasks. There is a rising need to investigate the tasks and programs of community health centers, which have been developing organically, and the appropriateness of their concerned space. The reality, however, indicates poor research efforts for the topic.
This study thus set out to analyze the task importance and appropriateness of space of current community health centers in response to the increasingly diverse projects and programs of community health centers according to local characteristics, providing basic data and guidance needed to make space plans for community health centers to provide local residents with better health service.
The study employed the survey technique for its methodology. A survey was conducted with 126 staffers at community health centers in Gyeonggi Province. It was accompanied by literature study on the information databases, papers, and publications including books home and abroad and also a primary preliminary study, which involved three staffers with a career of 20 years or longer at a community health center in a depth interview and classified the functions and spatial features of community health centers. The results were then used in questionnaire design, survey, and analysis in the order.
The scope and content of the present study encompassed papers, related books, and references in previous studies, investigations into the task importance and appropriateness of spatial composition of community health centers, analysis of questionnaire responses, and proposals for the working space of community health centers.
The analysis results of task importance show that they scored mean 96.15 points(3.85 points) out of 125(5) and that tuberculosis management recorded the highest task importance, being followed by anti-smoking projects, health education, and chronic diseases in the order. Checkup recorded the lowest level of task importance.
The appropriateness of space was low at mean 94.55 points(2.87 points) out of 165(5). The vaccination room recorded the lowest level of space appropriateness, being followed by the visiting healthcare room and the civil service room in the order.
The study then analyzed task importance and appropriateness of space according to the departments that the community health centers were currently running and found inverse correlations between task importance and appropriateness of space in medical examination, promotion of health, local healthcare, and management of infectious diseases. Disease control and administrative affairs were exceptions. Those findings indicate that some important tasks were given small space.
Those findings suggest that the community health centers made spatial plans without taking task importance into consideration. They will serve as basic data in future spatial plans at community health centers to allow for spatial allocation according to task importance and spatial planning for organic connections with the community.
