치매노인을 위한 빛의 치료적 효과 시각 및 생리학적 기능 - 지원을 위한 치료적 효과를 중심으로 - (A Study on the Therapeutic Effects of Light for Demented Elderly - focused on the visual and physiological therapeutic effects -)

초록

본 연구의 목적은 치매노인을 위한 시설적 환경이 단순 보호 제공 차원이 아니라 치매노인의 건강을 비롯한 삶의 기쁨 창출에 기여하는 치료적 환경이 될 때 절실하게 요구되는 빛의 중요성을 시각 및 생리학적 기능 지원 중심의 빛의 치료적 효과라는 인식을 통해 마련하는 것이다. 따라서 본 연구는 국내ž외 문헌고찰의 연구방법을 통해 빛과 치매노인의 시각 특성을 이론적으로 고찰하고 우리나라 치매노인을 위한 시설에서 쉽게 간과되고 있는 치매노인을 위한 빛의 치료적 효과를 치매노인의 시각기능 및 생리학적 기능의 지원 중심으로 살피며, 특히 치매노인의 생리학적 기능 지원 관련해 1990년대 이후 치매노인을 대상으로 이루어진 '빛 치료'(light therapy) 관련 국외 문헌자료를 중심으로 빛의 치료적 효과를 밝히고자 했다. 연구 결과를 정리하면 다음과 같다. 1)치매노인은 노인성 축동, 노안, 백내장, 녹내장, 노인성 황반 변성, 시력저하, 공간시각저하, 시야축소 및 색채 분별력 쇠퇴라는 고령자에서 나타나는 일반적인 시지각 기능의 노화 특성 외에 치매노인에서 더 특수하게 나타나는 시각 관련 이상증상으로서의 입체시력(stereoacuity), 대비감지력(contrast sensibility) 및 색채분별력(color discrimination)에 대한 감퇴 중심의 시각 특성을 나타냈다. 2)치매노인의 삶의 질적확대에 기여하는 빛의 치료적 효과는 치매노인의 시각 및 생리학적 기능의 지원에 있었으며, 특히 생리학적 기능 지원에서 빛은 치매노인의 일주기리듬 조절 및 비타민D 합성 작용을 지원하는 것으로 나타났다. 3)시각기능 지원과 관련해 빛은 치매노인의 공간인지 장애를 감소시켜 낙상사고를 방지하는 치료적 효과를 나타냈다. 4)생리학적 기능 지원과 관련해빛은 치매노인의 수면장애 및 격앙행동 중심의 행동장애와 우울증 중심의 심리장애를 완화시키는 효과를 나타냈다.

영어초록

The purpose of this study is for making the base of importance of light for demented elderly through the therapeutic effects of light which can be the most special element of importance in a therapeutic setting of dementia nursing facility contributing to create health and the enjoyment of life for demented elderly. Accordingly this study investigates through the internal and external literature examination the characteristics of light as well as the visual characteristics of demented elderly, and then classifies the therapeutic effects of light for demented elderly into both the visual and the physiological therapeutic effect which means that light supports the visual and the physiological function of demented elderly. Particularly the physiological therapeutic effect of light is to clarify through the external literature examination from the 'light therapy' for demented elderly since the 1990s. The results are as below. 1)The demented elderly represented both the general visual characteristics of the aged such as miosis, presbyopia, cataract, glaucoma, age-related macular degeneration (AMD), vision decline, visiospatial decline, reduction of the visual field and color discrimination, and the specific visual characteristics of the demented elderly such as the decline of stereoacuity, contrast sensibility and color discrimination. 2)The therapeutic effects of light for the demented elderly were the visual and physiological effects, and the latter acted as a biological clock supporting the circadian rhythm regulation and Vitamin D synthesis concretely.3)Regarding its visual therapeutic effect light took the effect to prevent fall and to decrease the degree of cognitive impairment like spatial cognition. 4)Regarding its physiological therapeutic effect light reduced the degree of behavioral disorders like sleep disorders and agitation, as well as the psychological disorder like depression.
