말기암환자의 통증과 불면증 감소를 위한 현실요법 프로그램 개발과 효과 연구 (A Study on the Healing and Development of Reality Therapy Program for Hospice)

초록

본 연구는 Glasser의 현실요법을 응용한 말기암 환자를 위한 호스피스의 통증 감소와 불면증 감소를 위한 새로운 호스피스 현실요법 프로그램을 개발하는 것이 연구목적이었다. 본 프로그램의 개발은 크게 2개 과정을 통하여 이루어졌다. 먼저 기존의 현실요법에 관한 문헌의 검토와 현실요법 자문가의 검토를 거쳐서 수정 보완, 확정하여 완성을하였다. 프로그램의 주요 내용은 현실요법의 주요 개념인 현실요법적인 상담환경 조성, 욕구탐색(Want), 행동탐색(Doing), 자기평가(Evaluation), 적용계획(Plan) 등으로 구성되었다. 본 프로그램의 특징은, 연구가들에 의해 그 효과가검증된 Glasser의 현실요법과 선택이론에 기초하였다. 프로그램 내용의 구성 차원에서, 현실요법과 선택이론의 핵심적인개념들이위계적이며체계적으로조직되고통합되었다. 프로그램 운영방법 차원에서, 현실요법과선택이론의핵심 내용이나 주장과 일치하도록 구성되어 있다. 배우고 익힌 내용을 즉시 말기암 환자들의 호스피스에 적용하도록 하여 참여자의 적용능력을 강조하였다. 본 프로그램의 효과에 대한 연구 설계는 사전-사후-추수검사 실험설계이다. 통증과 불면증으로 괴로워하는 호스피스 말기암 환자 10명을 대상으로, 3개월 간 매주 1회, 매 회기 50분, 총 8회에 걸쳐 프로그램을 진행하면서 통증 척도와 불면증 척도를 검증한 결과는 통증 감소와 불면증 감소가 있었으며, 이러한 효과는프로그램 종료 4주 후까지도 지속되는 것으로 나타났다. 결론적으로 본 연구에서 개발된 말기암 환자를 위한 현실요법 프로그램은 통증 감소와 불면증 감소에 효과적이었으며 앞으로 이 분야에서 유용하게 활용되기를 기대한다.

영어초록

This study was aimed to develop a new reality therapy (Hospice Reality Therapy: HRT) program for hospice to reduce pains and insomnia of terminal cancer patients by applying the reality therapy originated by Glasser and its efficacy of a newly developed program. The HRT program was developed through two processes in principle. First, the existing reality therapies contained in the literature were searched and reviewed by the expertise of reality therapy and counsellors for perfection through revision and complementation.
Major contents of the program were the composition of counselling environment, Want, Doing, Evaluation and plan, which were major concepts of the reality therapy. The specific features of the HRT program were as follows; First, it was based on the reality therapy and selection theory of Glasser, of which efficacy has been verified by researchers. Second, the core concepts of reality therapy and selection theory were hierarchically and systematically organized and integrated in terms of composition of program contents. Third, the operation method of the program coincides with core contents or issues of the reality therapy and selection theory.
Fourth, the contents that have been learned and practiced could be applied to hospice for terminal cancer patients to emphasize the applicability to participants. For a examination of the efficacy when using the HRT program, the research design used in this process was an experimental design of pre-test, post-test and follow up-test. And, the participants were tension, terminal cancer patients in an experiment group who had been suffering severely from pain and insomnia due to terminal cancer. The program was processed eight times in total, 50 minutes per time, once a week for three months to examine the pain scale and the insomnia scale.
This examination indicated pain reduction and insomnia reduction in the experimental group, which continued up to four weeks after the completion of the program. It was concluded that the HRT program developed in this study, a reality therapy program for terminal cancer patients, was effective for pain reduction and insomnia reduction. Furthermore, it is necessary to continue to use this program for health.
