초록

배 경 : 혈관통로 (vascular access)의 기능부전은 투석환자에서 이환율의 가장 중요한 원인이다. 최근 MIA (Malnutrition, Inflammation, Atherosclerosis) syndrome이 알려지면서 투석환자에서 죽상경화증 (atherosclerosis)과 염증간의 관련성이 부각되었다. 혈액투석환자들의 혈관통로 협착과 죽상경화증의 조직학적 유사성을 생각할 때 염증매개성 물질에 의한 만성염증반응이 죽상경화증을 촉진시키는 것처럼 혈관통로의 협착도 촉진시킬 수 있다는 가설을 세워볼 수 있다. 따라서 혈관통로협착이 혈청의 염증매개성 싸이토카인 (cytokine)과 관련성이 있는지 살펴보고자 하였다.
방 법 : 혈액투석을 받는 환자 72명을 대상으로 하여 ELISA를 이용하여 interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), interferon-γ (INF-γ)의 혈청농도 및 염증표지자로써 C-반응단백, ferritin농도를, 영양상태 표지자로써 알부민과 transferrin농도를 측정하였다. 또한 과거 진료기록을 검토하여 혈관통로협착을 조사하였다.
결 과 : 72명중 혈관통로협착은 35명에서 발생하였으며 평균 혈관통로 생존율은 22.7±37.6개월이었다 (평균 관찰기간 70.9±46.6개월). 환자들을 자가동정맥루를 사용한 환자군 (50명)과 인조혈관을 사용한 환자군 (22명)으로 나누어 분석하였을때, 자가동정맥루를 사용한 환자에서 세 가지 싸이토카인에 대한 다변량 Cox 회귀분석 결과 INF-γ가 의미있는 독립변수였다 (Hazard ratio 1.024, p=0.044). 또한 TNF-α는 ferritin이나 IL-6와 양의 상관을 보였고, IL-6와 C-반응단백도 양의 상관을 보였다. C-반응단백과 IL-6는 알부민과, transferrin은 ferritin과 음의 상관을 보였다.

영어초록

background : Histologically, vascular access stenosis is similar to the atherosclerotic lesion such as neointimal hyperplasia. However, the pathogenesis is not completely understood. The development of Inflammation and atherogenesis in hemodialysis population has been well recognized and is known as the malnutrition-inflammation-atherosclerosis (MIA) syndrome. We, therefore, examined the relationship between the vascular access stenosis and proinflammatory molecules in hemodialysis patients
Methods : Seventy-two regularly dialyzed patients were observed and the serum levels of the proinflammatory cytokines (IL-6, TNF-α and INF-γ) were measured by ELISA. Patients were followed until the earliest date of death, modality change or loss to follow up.
Results : Vascular access stenosis was developed in thirty-five patients, and their mean duration of access survival was 22.7±37.6 months. In thirty- seven patients, vascular access remained patent during the study period (mean follow up; 70.9±46.6 months). In patients with autologous AV fistula, multivariate Cox regression analysis showed a significant contribution of INF-γ to vascular access stenosis (Hazard ratio 1.024, p=0.044). In patients with AV graft, there was no significant relation between inflammatory cytokines and graft stenosis. There were positive correlations between the level of TNF-α and ferritin, and between transferrin and albumin. CRP and ferritin were negatively correlated with albumin and transferrin.
Conclusions : Markers of inflammation were significantly associated with nutrition. And INF-γ, one of the inflammatory cytokines, was related to the vascular access stenosis.
