초록

개화기 한국 사회의 건강수준 및 질병양상을 객관적으로 보여줄 수 있는 사료로 제중원에서 근무한 서양인 의사들이 1886년과 1901년에 작성한 두 개의 보고서가 존재한다. 하나는 알렌과 헤론이 작성한 ‘조선정부병원 제1차년도 보고서’이고, 또 다른 하나는 에비슨이 작성한 ‘1901년도 대한제국병원 연례보고서’다. 물론 제중원이 라는 단 한 개의 의료기관을 이용한 환자를 대상으로 기록한 것이기에 개화기 조선 전체의 질병 양태를 파악하기에는 한계가 있다. 그럼에도 제중원의 진료기록보다 당시 조선의 질병 발생 상황을 구체적으로 알 수 있는 자료는 없다. 더 나아가서는 당시 서양의학에 기대감이 컸던 질병, 달리 말해서 전통의료에서 해결해 주지 못했 던 질병들에 대해 파악해볼 수 있는 단초를 제공한다. 이를 정리해 보면, 말라리아를 비롯한 각종 발열병, 소화기병, 매독을 비롯한 성병, 옴을 비롯한 각종 피부병, 눈의 질병 등이 당시 조선인들에게 많았음을 알 수 있다. 한편 제중원의 서양의술을 통해 가장 큰 치료 효과를 거둔 질병은 말라리아, 눈의 질병, 각종 농양, 외상, 항문병 등이었다. 또한 이를 조선 민중들의 입장에서 본다면, 대략 키니네와 같은 별도의 치료약이 존재하는 경우와 외과수술이 필요한 경우에 제중원을 찾았음을 알 수 있 다. 아울러, 1886년 보고서에 비해 1901년 보고서에서 여성 환자 비율이 크게 증가하 고 있어 개화기 조선 사회의 서양의학에 대한 인식 변화를 유추해볼 수 있다.

영어초록

As historical materials that show objectively the health levels and disease aspects of Korean society during the period of enlightenment, there are two reports prepared in 1886 and 1901 by Western doctors who worked for Jejungwon hospital. One is ‘First Annual Report of the Korean Government Hospital’ written by Allen and Heron, and the other is ‘1901 Annual Report of the Imperial Korean Hospital’ written by Avison. Of course, they have limitations in showing the disease aspects of the whole Joseon in the period of enlightenment, because they analyzed clinical statistics at only one medical institution called Jejungwon. From them, however, we can see the glimmer of aspects of the then disease distribution in the central region around Hanseong. Further, they provide a clue to diseases the remedy of which was much expected from the then Western medicine, in other words, diseases that traditional medicine could not treat. To sum up from this viewpoint, records on the treatment of outpatients and inpatients found in the two reports issued by Jejungwon show that Joseon people in Hanseong and its neighboring central region suffered much from various diseases with fever including malaria, gastroenteric diseases, sexually transmitted diseases including syphilis, various skin diseases including scabies, and ocular diseases. On the other hand, it is also found that the Western medicine at Jejungwon had the greatest curative effects for malaria, ocular diseases, various abscesses, physical trauma, and anal diseases. To view this from the viewpoints of the general public, it can be seen that they generally visited Jejungwon in case that Jejungwon had a specific therapeutic medicine such as quinine or that they needed surgery. In addition, the ratio of female patients increased greatly in the 1901 report compared with the 1886 report, from which it can be inferred that there was a change of perception on the Western medicine in the Joseon society during the period of enlightenment.
