초록

목적: 본 연구는 지역사회에서 독립적인 보행이 가능한60세 이상의 만성 뇌졸중 환자의 일반적 특성, 인지 기능과신체 기능 간의 상관성을 분석하여 효과적인 복합중재 프로그램을 개발하는데 기초자료로 활용하고자 하였다.
방법: 다음의 선정기준을 만족하는 대상자를 선별하여인지 기능과 신체 기능을 측정하였다. 선정기준은 첫째, 60 세 이상이며, 뇌졸중으로 진단받고 발병일이 2년 이상인자 둘째, 수정바델지수(MBI) 점수가 75점 이상이며, 지역사회에서 독립보행이 가능한 자 셋째, 직선이등분검사에서 편측무시가 없으며, 지시수행이 가능한 자이다. 인지 기능 검사는 K-MoCA를 시행하였으며, 신체 기능 검사는 정적균형검사인 Romberg, Semi-Romberg, Tandem-Romberg 검사를진행하였고, 동적균형검사는 4m 보행속도 검사, 5회 의자에서 일어서기 검사, 일어서서 걷기 검사(TUG)를 시행하였다. 자료분석은 Spearman’s correlation을 시행하였다(p<.05).
결과: 뇌졸중 환자의 인지 기능(K-MoCA)은 4m 보행속도는 r=-.564로 다소 높은 상관관계를 보였으며, 4m 보행속도와 TUG는 r=-.787로 높은 음의 상관관계를 보였다(p<.05).
결론: 본 연구의 결과 지역사회 뇌졸중 환자의 인지-신체복합중재 프로그램을 제시할 때 인지기능이 동적균형 능력과 상관관계가 있음으로 복합중재 세부 프로그램을 개발하는데 근거자료로 활용될 수 있을 것으로 생각된다.

영어초록

This study aimed to use basic data to develop a comprehensive intervention program to effectively improve cognitive and physical functions in individuals ≥ 60 years of age who experienced chronic stroke and are capable of independent walking in the community. This study analyzed the general characteristics of and correlations between cognitive and physical functions to develop the program. Subjects included individuals who voluntarily expressed their willingness to participate in the clinical trial from the recruitment notice posted at N institution (Seoul, South Korea) and fulfilled the following selection criteria: age≥60 years and diagnosed with stroke, with an onset of at least 2 years; Modified Barthel Index score≥75, indicating that a mild level of assistance was required and capable of independent mobility in the community; and absence of neglect in the line bisection test and able to follow test instructions. As a result, of the 23 individuals recruited, 16 were ultimately included. The Korean version of the Montreal Cognitive Assessment (i.e., “K-MoCA”) was used to assess cognitive function. Physical function was assessed using static balance tests, including the Romberg, semi-Romberg, and tandem Romberg tests. Dynamic balance tests included a 4 m walking speed test, a 5-time chair stand test, and Timed Up and Go (TUG) test. Data analysis was performed using SPSS version 22.0 (IBM Corporation, Armonk, NY, USA), and Spearman's rank-order correlation was used for correlation analysis. The cognitive function of patients with stroke exhibited a slight correlation with 4 m walking speed (r=-.564), while 4 m walking speed and TUG exhibited a negative correlation (r=-.787) (p<.05). Based on these findings, when presenting a comprehensive cognitive-physical intervention program for community-dwelling stroke patients, it may be inferred that there is a correlation between cognitive function and dynamic balance ability. This can serve as evidence supporting the development of detailed programs for comprehensive intervention(s).
