뇌질환 환자에서 전산화 인지치료의 임상적 유용성: 체계적 고찰 및 메타분석 (Clinical Benefit of Computerized Cognitive Therapy in Patients with Brain disease : A systematic review and meta-analysis)

초록

Purpose: 본 연구는 국내 뇌질환 대상자에 대한 전산화 인지치료의 임상적 유용성의 확인과 향후에 재활관련 전문가들에게 전산화 인지치료의 효과성을 확인할 수 있는 기초자료를 제시하고자 한다.
Methods : 국내 뇌질환 환자에게 적용한 전산화 인지치료의 임상적 효과성을 확인하기 위해 체계적 고찰 및 메타분석 방법을 사용하였으며 논문 검색 및 선정 방법은 PICOST-SD를 사용하였다.
Results : 최종 선정된 문헌들의 내용들을 종합적으로 분석한 결과, 뇌졸중과 치매에 대한 연구가 주로 이루어졌음을 알 수 있었다. 그리고 문헌의 질 평가를 통해 기존 전산화 인지치료의 연구들의 대부분이 양질의 연구로 파악되었지만 무작위 할당과 맹검에 대해서는 부족한 결과를 나타내었다. 또한, 메타분석을 통해 뇌질환 환자의 인지기능과 일상생활활동, 시지각 기능에 대해 전산화 인지치료의 임상적 효과성을 알 수 있었다.
Conclusion : 기존 뇌질환 대상자들에게 전산화 인지치료를 적용한 연구 문헌의 내용들을 종합적으로 분석하여 파악함으로써 향후 뇌질환 환자들에게 중점적으로 적용 되어야 인지치료 특성과 전산화 인지치료의 실제 임상에서의 유용성에 대한 기초 근거 자료로 활용 될 수 있을 것이라고 생각된다.

영어초록

Purpose: This study aims to confirm the clinical usefulness of computerized cognitive therapy for patients with brain diseases in Korea and to present basic data that can confirm the effectiveness of computerized cognitive therapy to experts related to rehabilitation in the future.
Methods : A systematic review and meta-analysis method was used to confirm the clinical effectiveness of computerized cognitive therapy applied to patients with brain diseases in Korea. And PICOST-SD was used to research and select papers.
Results : As a result of comprehensive analysis of the contents of the finally selected literature, it as found that studies on stroke and dementia were mainly conducted. In addition, through the evaluation of the quality of the literature, most of the existing computerized cognitive therapy studies were identified as high-quality studies, but the results were insufficient for randomization and blinding. And through meta-analysis, the clinical effectiveness of computerized cognitive therapy on cognitive function, activities of daily living, and visual perception function of patients with brain disease was found.
Conclusion : The research literature that applied computerized cognitive therapy to brain disease subjects was comprehensively analyzed and identified. It is thought that this could be used as basic data on the characteristics of cognitive therapy that should be applied to patients with brain diseases in the future and the usefulness of computerized cognitive therapy in actual clinical practice.
