재가 뇌졸중 환자의 일상생활동작 향상을 위한 방문물리치료의 비용-효용 분석 (Cost-utility analysis of home physical therapy to improve daily activities of stroke patient living at home)

초록

목 적 : 본 연구는재가 뇌졸중 환자를 대상으로 하는 두 가지 물리치료 방법인 병원 내원 물리치료와 방문물리치료의 비용-효과 비율을 비교하고 방문물리치료의 효과에 대한 경제적 타당성을 규명하고자 하였다.
방법 : 본 연구에서는 뇌졸중 환자에 대한 두 가지 물리치료 방법의 비용과 효과를 측정하기 위해 병원 내원 물리치료와 방문물리치료 두 가지 방법을 비교하여 대상자를 모집하였다. 수집된 전체 데이터 중 82명의 참가자가 병원 내원 물리치료, 90명이 방문물리치료를 위해 선정되었다. 비용을 측정하기 위해 병원 내원 물리치료의 경우 환자, 가족, 의료기관, 정부의 직접비용과 간접비용을 측정하였고, 방문물리치료의 경우 환자, 가족의 직접비용과 간접비용, 방문물리치료 기관 및 정부의 비용이 계산되었다. 효과를 측정하기 위해 두 가지 방법 모두에서 일상생활동작을 측정하였다. 수집된 데이터를 통해 비용효과비와 점증비용효과비를 분석했다.
결 과 : 방문물리피료가 비용효과비 분석에서 병원 내원 물리치료에 비해 비용효과성이 낮았고, 점증비용효과비에서도 차이를 보여 방문물리치료가 비용 대비 높은 효과를 나타낼 수 있음을 시사하였다.
결 론 : 재가 뇌졸중 환자의 물리치료 방법을 선택함에 있어 기존의 다른 물리치료 방법에 대한 대안으로 방문물리치료가 고려될 수 있을 것으로 사료된다. 또한, 본 연구의 결과는 방문물리치료를 제도화하기 위한 정책적 결정에 있어 방문물리치료가 경제적인 혜택뿐만 아니라 재가 환자 치료에 더욱 효과적일 수 있다는 근거를 제공하는데 기여할 것이다.

영어초록

Purpose : This study tried to compare cost-effectiveness ratio of physical therapy in health centers and home physical therapy, the two methods of physical therapy for home-bound stroke patients and clarify the economic validity about the effect of home physical therapy.
Methods : In this study, to measure the cost and effectiveness of two physical therapy methods for stroke patients, subjects were recruited by comparing two methods, in-hospital physical therapy and home physical therapy. Among the whole data that were collected, 82 participants were selected for in-hospital physical therapy and 90 for home physical therapy. To measure costs, in terms of in-hospital physical therapy, direct cost and indirect cost for patients, family, medical institutes, and the government were measured, and in terms of home physical therapy, direct cost and indirect cost for patients, family, institutes for home physical therapy, and the government were calculated. To measure effectiveness, activities of daily living were measured in both method. Through collected data, cost-effectiveness ratio and incremental cost-effectiveness ratio were analyzed.
Results : The analysis of cost-effectiveness of the two measures home physical therapy showed low cost-effectiveness than in-hospital physical therapy and incremental cost-effectiveness ratio also showed difference, implying home physical therapy could have high effectiveness compared to cost.
Conclusion : Through such results, it is believed that in selecting ways of physical therapy for home-bound stroke patients, home physical therapy will be considered as an alternative to other existing methods of physical therapy. In addition, in policy decisions to make home physical therapy established as a system, the result of this study will contribute to providing grounds that home physical therapy not only offers economic benefits but can be more effective in treating home-bound patients.
