상완골 대결절 단독 골절의 다양한 수술 방법에 따른 임상적 결과 (Clinical Results of Various Surgical Techniques for Isolated Fracture of Greater Tuberosity of Humerus)

초록

목적: 상완골 대결절 단독 골절에 대한 다양한 수술 방법에 따른 임상적 및 방사선학적 결과에 대해 비교 분석해 보고자 하였다.
대상 및 방법: 2001년 2월부터 2008년 12월까지 상완골 대결절 단독 골절로 진단받고 수술을 받은 33명의 환자 중 1년 이상 추시가 가능했던 31명의 환자를 대상으로 하였다. 수술 방법은 비흡수성 봉합사를 이용한 경골 고정술(transosseous sutures)(16예), 유관 나사(cannulated screw)를 이용한 고정술(10예), 긴장대 고정술(2예), 골편 제거술 및 회전근 개 봉합술(2예) 및 경피적 핀 고정술(1 예)을 각각 시행하였다.
결과: 최종 추시 시 평균 Constant 점수는 79.4점이었고, Korean Shoulder Score (KSS) 점수는 81.2점이었다. 시행된 술식 중 경골 고정술을 시행한 경우가 Constant 점수 82.5점, KSS 점수 89.3점으로 가장 높았다. 골유합은 평균 10.3주(7-15주)에 얻을 수 있었고, 내고정 실패로 재수술을 했던 경우가 2예 있었다. 견관절 강직이 발생한 3예는 모두 삼각 흉근 도달법을 이용한 경우에서 발생하였다.
결론: 상완골의 대결절 단독 골절 시 다양한 수술 방법 및 술기를 통하여 임상적 및 방사선학적으로 양호한 결과를 얻을 수 있었고, 그 중 경골 고정술이 상대적으로 우수한 임상적 결과를 보였다.

영어초록

Purpose: To compare the clinical and radiologic outcomes of various surgical techniques for an isolated fracture of greater tuberosity of the humerus.
Materials and Methods: From February 2001 to December 2008, 31 patients, who underwent an operation for isolated greater tuberosity fracture and were followed up for more than 1 year, were enrolled in this study. The mean age at the time of operation was 49.3 years (range, 23-73 years). The operation methods included in this study were as follows: a transosseous suture using nonabsorbable suture material (16 cases), a fixation by cannulated screws (10 cases), tension band wiring (2 cases), bony fragment excision with rotator cuff repair (2 cases), and percutaneous pinning (1 case).
Results: At the last follow-up, the average Constant score was 79.4 and Korean Shoulder Score (KSS) was 81.2. Among the various operation methods used in this study, the transosseous suture had the highest scores with 82.5 in Constant score and 89.3 in KSS. Bone union was achieved at average 10.3 weeks (range, 7-15 weeks), and there were 2 cases in which the reoperation was required due to internal fixation failure. Postoperative shoulder stiffness occurred in 3 cases, and all the cases were done with the deltopectoral approach.
Conclusion: Clinically and radiologically satisfactory results were obtained using various operation techniques for an isolated greater tuberosity fracture of the humerus. The transosseous suture showed relatively better results than the other methods used in this study. To achieve favorable clinical and radiologic results, it is important to select an appropriate surgical approach and fixation method according to the fracture site, degree of displacement, and size of fragment.
