가속도계 최소 착용기준에 따른 좌업활동시간 추정값의 신뢰도 및 구인타당도 근거 (Estimates of Accelerometry-Measured Sedentary Behavior: Validity and Reliability Evidences based on Different Wear-Time and Day Criteria)

초록

본 연구는 가속도계를 이용하여 좌업활동량을 측정할 때 최소한의 착용기준(착용시간 및 착용일수)에 따른 결측값발생 정도를 알아보고, 착용기준에 따른 좌업생활량 추정값의 신뢰도를 알아보며, 구인타당도 근거를 제시하는 목적으로 수행되었다. 연구참여자는 NHANES 2005-2006 대상자 중 가속도계를 착용한 18-64세 성인 3,658명이었으며, 남성 참여자 비율은 46.05%였다. 안정성 신뢰도는 최소 착용시간 기준별 하루치 좌업활동량을 기준으로 한 급내상관계수(ICC), 그리고 최소 착용일수가 증가함에 따른 신뢰도 증가 예측값을 근거로 하였다. 구인타당도 근거는 각 착용기준별 좌업활동량과 대사증후군 지표와의 관계를 제시하였다. 결측값이 전체 참가자의 20% 미만인 착용기준 중ICC>0.9인 경우는 주말을 최소 하루 포함할 경우 2일 10시간 이하, 3일 8시간 이하, 4일 6시간 이하였다. 주말을 제외할 경우는 1일 9~12시간, 2일 10시간 이하, 3일 8시간 이하, 4일 6시간 이하였다. 이들 중 허리둘레와 좌업활동량사이의 유의한 선형적 관계를 보인 착용기준은 주말 포함할 경우 최소 4시간, 2~3일(trend-p=.022-.028), 주말을 제외할 경우 1~3일, 4~10시간이었다(trend-p=.022-.045). 한편 중성지방과 유의한 관계를 보인 착용기준은 주말 포함할경우 모두(trend-p=.003-.024), 주말을 제외할 경우 1-4일, 4-10시간 이었다(trend-p=.019-.047). 본 연구 결과 착용시간 및 일수 기준을 엄격하게 적용할수록 결측값이 많이 발생하는데 비해 신뢰도 증가는 미미했음을 보여주었다. 그리고 주말을 포함할 경우 최소 이틀치 네 시간 이상, 주말을 제외할 경우 최소 하루치 네 시간 이상 가속도계 자료를 확보하면 좌업활동량을 충분히 안정적으로 추정할 수 있는 것으로 나타났다. 끝으로 신뢰도가 충분히 담보될 경우, 일부측정기준에 따른 좌업활동량 추정값과 대사증후군 지표 중 허리둘레 및 중성지방 사이에 유의한 선형관계가 있는 것으로 밝혀졌다. 이는 각 측정기준에 따른 좌업활동량 추정값의 구인 타당도 근거를 일부 제시한 의의가 있다.

영어초록

The purposes of this study were 1) to investigate patterns of missing value based on different accelerometer wear-time and day criteria, and 2) to provide reliability and validity evidences of estimates of accelerometry-measured sedentary behavior. The study participants were 3,658 US adults who wore Actigraph 7164 in the NHANES 2005-2006. Stability reliability of different wear-day criteria were estimated using intraclass correlation coefficient (ICC) and Spearman-Brown Prophecy formula for each wear-time criterion. Statistical significance tests of the linear associations between indices of metabolic syndrome and wear-time and day criteria were performed to provide construct validity evidences. Results showed that 2-day-10-hour or less, 3-day-8-hour or less, 4-day-6-hour or less minimum wear-time and day (including at least a weekend day) criteria rendered less than 20% missing values and >.9 ICCs.
When weekend was excluded, the criteria were 1-day-9- to 12-hour, 2-day-10-hour or less, 3-day-8-hour or less, 4-day-6-hour or less. Among these criteria, when at least a weekend day was included, 4 or more days of 2- to 3-day of wear-time and day yieled significant associations between sedentary time and waist circumference (trend-p=.022-.028). When weekend was excluded, 1- to 3-day of 4- to 10-hour criteria yielded significant associations (trend-p=.022-.045). All wear-time and day criteria that included at least a weekend day rendered significant associations between sedentary time and triglycerides (trend-p=.003-.024). When weekend was excluded, 1- to 4-day of 4- to 10-hour criteria rendered significant associations. The results of this study suggest that strict application of wear-time and day criterion increased missing values, but the stability reliability did not improve substantively. In addition, 2-day of 4-hour or more (including at least a weekend day) or 1-day of 4-hour or more (excluding weekend days) provided sufficiently stable estimates of sedentary time. When reliability was high, this study provided a part of construct validity evidences for selected wear-time and day criteria.
