일본과 독일 장기요양보험 청구체계의 현황과 시사점 (A study on the claiming system in long-term care insurance in Japan and Germany)

초록

본 연구는 일본의 개호보험과 독일의 수발보험을 중심으로 장기요양급여 청구체계의 기본 구조와 현황을 살펴봄으로써, 장기요양 청구체계의 과제를 도출하고 향후 제도 개선 논의를 위한 정책적 시사점을 제공하고자 하였다. 연구를 위해 양국의 장기요양보험 청구체계 관련 제도와 업무현황를 중심으로 문헌 및 사례조사, 실무자 인터뷰를 실시하였다. 일본과 독일은 저마다 사회문화, 제도적 환경의 특성을 반영한 청구체계를 운영하고 있으며, 변화하는 서비스 및 기술에 따라 지속적인 개선 노력을 기울이고 있다. 청구체계 및 시스템 개발과 운영에 공공만이 아니라 민간 사업자의 참여를 허용하고 있으며, 이때 사회보험제도 하에서 기본적으로 준수해야할 기준과 절차를 명확히 제시하고 있다. 특히 장기요양서비스의 계획과 제공, 청구와 심사에 이르는 업무의 연결성 속에서 청구체계를 구축하여 운영하고 있으며, 최신 정보기술을 청구시스템에 도입하여 활용하는 등 지속적인 개선을 시도하고 있다. 우리 사회도 노인장기요양보험 청구체계의 개선을 위해 장기요양현장의 청구업무 역량을 강화하고 이를 지원하기 위한 효율적인 청구상담 및 지원체계를 구축하는 한편, 이용자 중심의 청구시스템을 개발하고 전체 제도의 관점에서 청구체계와 실제 이용절차간의 통합성을 확보할 필요가 있다.

영어초록

This study aimed to generate the tasks of the claiming system in long-term care insurance and to provide the policy implication for further discussion of the system improvement, upon reviewing the basic structure and status of the claiming system focusing on long-term care insurances in Japan and Germany, where are prior experienced countries, in view of the development of senile long-term care insurance system and securing its sustainability as the social insurance. For the study, literature reviews, case studies and face-to-face interviews with the persons in charge were performed on the system and the status related to the claiming system of the long-term care insurance in Japan.
Japan and Germany where introduced and operated the long-term care system with the social insurance type, similar to the Korean system, prepared and operated the proper claiming system according to their own sociocultural and systemic environmental characteristics, and they tried the seamless improvements upon the changes of environment and technologies. Not only public sectors including the nation and the insurers but also the private sectors are allowed to participate in the development and operations of the claiming system, and the standards and procedures that should be complied are suggested under the social insurance system. Especially applying the characteristics of long-term care services, the claiming system was developed and operated in a series of processes from service planning and provision up to claiming and reviewing, and continuous efforts were made to utilize the recent information technology introducing to the claiming system actively.
Our society requires to strengthen the competency of claiming work at the site of long-term care services, to develop more efficient claiming consulting and support system to assist this, to develop user-friendly claiming system, and also to secure the integration of the claiming system with the actual processes in view of the integrated system.
