치매환자 및 치매환자 보호자의 특성과 치매환자 보호자 삶의 질에 관한 연구 (A Study on Characteristics of People with Dementia and Their Family Caregivers and the Quality of Life for Family Caregivers)

초록

목적 : 본 연구에서는 치매환자 보호자의 삶의 질 평가도구를 이용하여 치매환자 및 치매환자 보호자가 가지고 있는 일반적인 특성들이 치매환자 보호자들의 삶의 질에 어떠한 영향을 미치고 있는지 알아보고자 한다.
연구방법 : 치매로 진단 받은 치매환자의 가족들을 대상으로 2022년 3월부터 동년 11월까지 치매환자와 관련된 7개 항목, 치매환자 보호자와 관련된 12개 항목 및 치매환자 보호자 삶의 질 평가 23개 항목으로 구성된 설문지를 통해 자료를 수집하였다. SPSS 25.0 version을 사용하여 치매환자와 치매환자 보호자의 일반적 특성을 확인하기 위해 기술통계를 사용하였으며, 일반적 특성에 따른 차이는 독립표본 t 검정과 일원배치 분산분석을 시행하였다.
결과 : 연구에 참여한 치매환자 보호자 삶의 질 평균 점수는 46.92점이었으며, 하부 영역별 점수는 전반적인 건강상태 7.89점, 일상생활 5.54점, 업무 및 기타 활동 13.81점, 부담감 10.14점 및 스트레스 9.54점으로 확인되었다. 치매환자 보호자 삶의 질에 영향을 미칠 수 있는 일반적 특성으로 치매환자가 여성인 경우, 기능적 수준이 낮을수록, 문제행동 중 기억력 저하로 인한 행동, 날짜·시간 등에 대한 인식 저하, 대·소변 처리와 관련된 문제행동, 충동적인 행동, 의심·불안·망상이 있는 경우 치매환자 보호자 삶의 질이 저하되는 것으로 나타났다. 또한 치매환자 보호자의 경제적 수준이 낮을수록, 치매환자와의 관계가 맏며느리, 며느리, 딸 관계일 경우 치매환자 보호자의 삶의 질이 저하되는 것으로 확인되었다.
결론 : 본 연구를 통해 치매환자 보호자 삶의 질에 영향을 미치는 치매환자 및 치매환자 보호자의 주요 요인들이 확인되었다. 광역치매센터, 치매안심센터, 주간보호센터 등 다양한 기관에서 치매환자 보호자를 위한 교육, 상담 및 중재 프로그램 개발과 함께 치매환자 보호자를 위한 정책 개선에 기여할 수 있기를 기대한다.

영어초록

Objective : In this study, we aimed to find out how the general characteristics of people with dementia and their family caregiver’s general characteristics affect the quality of life of family caregivers of people with dementia using a quality of life assessment tool for family caregivers of people with dementia.
Methods : Data were collected from families of dementia patients diagnosed with dementia through a questionnaire consisting of 7 items related to the people with dementia, 12 items related to the family caregivers of people with dementia, and 23 questions assessing the quality of life of the family caregivers of people with dementia, targeting family members of dementia diagnosed from March 2022 to November of the same year. Using SPSS 25.0 version, an independent samples t test and one-way analysis of variance were performed to identify differences in the general characteristics and general characteristics of people with dementia and their family caregivers.
Results : The average quality of life score of the family caregivers of people with dementia who participated in the study was 46.92, and the scores for each subarea were 7.89 for health condition, 5.54 for daily life, 13.81 for work and others, 10.14 for burden, and 9.54 for stress. General characteristics that can affect the quality of life of family caregivers of people with dementia include: if the people with dementia is female with the lower the functional level, the problem behavior due to memory decline, decreased awareness of date and time, and difficulty in handling stool and urine. It was confirmed that the lower the economic level of the family caregivers of people with dementia, the lower the quality of life of the family caregivers of people with dementia, if the relationship with the people with dementia was that of the eldest daughter-in-law, daughter-in-law, or daughter.
Conclusion : Through this study, major factors affecting people with dementia and their family caregivers' quality of life were identified. I hope that various organizations will be able to contribute improving policies for family caregivers of people with dementia along with developing education, counseling, and intervention programs for family caregivers of people with dementia.
