초록

연구 목적: 본 논문에서는 일본 고령사회기의 개호문학 작품 『황락(黃落)』과 『개호입문(介護入門)』을 토대로 2000년 4월 시행된 개호보험법을 검토하고 개호보험법 이전과 이후의 고령자 복지제도를 검토하고자 하였다. 아울러 검토를 통하여 개호보험법의 실효성과 한계를 규명하는 것을 연구목적으로 하였다.
연구 방법: 연구방법으로는 개호보험법 제정 이전의 작품 『황락』과 개호보험법 시행 이후의 작품 『개호입문』을 비교 분석하고, 작품 속에 기록된 개호사(介護史)를 통하여 개호보험법 시행 전후의 복지서비스 수요와 공급의 관계를 살펴봄으로써 개호보험법의 실효성과 한계를 규명하고자 하였다.
연구 내용: 연구를 통해 개호보험법은 이전의 제도와는 달리 수급자를 요지원 2단계와 요개호 5단계로 구분하고, 직업 개호사 및 케어 매니지먼트 제도를 도입하여 활용하고 있음을 알 수 있었다. 또한, 개호보험법의 시행을 통해 노인복지서비스의 수준이 높아지고 서비스 수급 체계가 명확해지는 등, 제도의 실효성이 상당 부분 입증되었다는 사실을 알 수 있었다.
결론 및 제언: 연구결과 개호보험법의 등장으로 갑자기 늘어난 직업 개호사의 질적 문제와 개호인의 심리 상태조사 부족, 개호인에 대한 정책적 지원 부족 등은 개호보험법의 또 다른 한계로 파악되었다.

This paper reviews the Nursing Care Insurance Act enforced in April 2000 based on the long-term care literature works Koraku and Kaigonyumon, the works of Nursing care literature during the aged society period in Japan, and the welfare system for the elderly before and after the Nursing Care Insurance Act. In addition, the purpose of this study was to identify the effectiveness and limitations of the Nursing Care Insurance Act through review. As a research method, we compare and analyze the work Koraku before the enactment of the Nursing Care Insurance Act and Kaigonyumon after the enactment of the Nursing Care Insurance Act, and examine the relationship between the supply and demand of welfare services before and after the enactment of the Nursing Care Insurance Act through the care history recorded in the work. By examining this, the effectiveness and limitations of the Nursing Care Insurance Act were investigated. Through the research, it was found that the Nursing Care Insurance Act classified the recipients' stages into 2 stages of required support and 5 stages of long-term care, and that the care manager and care management system were introduced and utilized. As a result, the level of welfare services for the elderly increased and the system of service supply and demand became clear, thereby proving the effectiveness of the system to a certain extent. On the other hand, with the advent of the Nursing Care Insurance Act, the quality problems of caregivers improvised, the limitations of home care to care for those who are in the 5th level of Nusing care, and the insufficient status check for the main caregivers who need to continuously care for them. This was pointed out as a limitation of the ‘Nursing Care Insurance Act.’
