초록

이 연구는 편측무시 환자들에게 치료적 음악활동(리듬악기 연주, 키보드 연주, 가사 보고 노래 부르기)을 실시하여 각각의 음악활동이 편측무시의 정도에 어떠한 영향을 미칠 것인가를 알아보기 위한 목적으로 이루어졌다. 연구의 대상은 노인전문병원에서 뇌졸중의 진단을 받고 장기입원 하에 치료를 받고 있는 좌측 편마비 환자들로서, 심한 편측무시가 있는 남녀 2명이었다. 실험은 2005년 9월 12일부터 11월 8일까지 약 9주간에 걸쳐 총 30회(전, 후 평가 3회 포함, 매회 40분간) 중다중재설계 방법으로 시행되었다. 연구는 기초선 기간과 중재의 순서로 진행되었으며, A는 기초선, B는 리듬악기 연주, C는 키보드 연주, D는 가사 보고 노래 부르기였다. 순서에 의한 영향을 통제하기 위해 대상자 1은 A1-
B-A2-C-A3-D의 순서로, 대상자 2는 A1-C-A2-D-A3-B의 순서로 시행하였다. 대상자들의 편측무시 감소 정도를 측정하기 위해 실험 첫 회기와 마지막 회기에 MVPT 검사를 실시하였다. 또한 매 회기마다 편측무시의 정도를 알아보기 위하여 Albert Test와 선 나누기 검사를 실시하였다. 이 연구에서 나타난 결과는 첫째, 치료적 음악활동은 대상자들의 편측무시 감소에 영향을 미치는 것으로 나타났다. 치료 후 대상자 1의 MVPT 원 점수는 21점에서 29점으로 시지각 능력에 향상을 보였으며, 좌측반응점수도 10점에서 16점으로 편측무시에 28.57%의 감소를 보인 것으로 나타났다. 대상자 2도 음악치료 프로그램을 시행한 후, 원 점수 20점에서 26점으로 시지각 능력에 향상을 보였으며, 좌측반응점수도 9점에서 14점으로 편측무시에 있어서 23.8% 감소를 보인 것으로 나타났다. 둘째, 각 음악활동은 기초선 기간에 비해 편측무시 감소에 영향을 미친 것으로 나타났다. Albert Test와 선 나누기 검사를 통해 나타난 대상자들의 편측무시 감소율의 평균을 보면, 중재 B(리듬악기 연주)는 23.03±4.24%, 중재 C(키보드 연주)는 35.82±8.88%, 중재 D(가사 보고 노래 부르기)는 26.82±4.12%의 감소를 보였다. 따라서 세 가지 중재 모두 편측무시 감소에 영향을 미친 것으로 나타났다.

영어초록

This study was conducted with the purpose of determining the effect of therapeutic music activities(playing rhythmic instruments, playing keyboards, and singing with written lyrics) on the degree of unilateral neglect of patients with left hemiplegia, who are not able to recognize the left side due to the disability induced by structural brain lesion. The subjects of this study included 1 male and 1 female inpatients who were diagnosed with a stroke at a hospital for the elderly receiving treatment for severe unilateral neglect due to left hemiplegia. The study was carried out from September 12 to November 8, 2005 for 9 weeks using the multi-treatment design with a total of 30 sessions which included 3 sessions of pre- and post-evaluations, 40 minutes per session. The study was conducted in the order of baseline period and intervention. The baseline was designated as A, rhythmic instrument playing was designated as B, keyboard playing was designated as C, and singing with written lyrics was designated as D, and in order to control the effects of order of the study, subject 1 was conducted in the order of A1-B-A2-C-A3-D and subject 2 was conducted in the order of A1-C-A2-D-A3-B. The MVPT test was used at the first and last sessions of the study in order to measure the level of decrease in unilateral neglect of the study patients. In addition, Albert test and line bisection test were given to the patients at each session in order to assess the level of unilateral neglect. The results of the study are as follows. First, therapeutic music activities showed an effect on decrease in the level of unilateral neglect of the patients. After the intervention, the MVPT raw score of subject 1 increased from 21 points to 29 points, showing an enhancement in his visual perception and he also displayed a 28.57% reduction in unilateral neglect with an increase in the left reaction score from 10 points to 16 points. Subject 2 also displayed improvement in her visual perception following music therapy program with an increase in the MVPT raw score from 20 points to 26 points and a 23.8% reduction in unilateral neglect with an increase in the left reaction score from 9 points to 14 points. Second, each music activity showed an effect on decrease in unilateral neglect compared to the baseline period. In evaluating the average reduction rate of unilateral neglect of the subjects that were found through the line bisection test, intervention B(rhythmic instrument playing) showed a reduction
of 23.03±4.24%, intervention C(keyboard playing) showed a reduction of 35.82±8.88%, and intervention D(singing with written lyrics) showed a reduction of 26.82±4.12%. Therefore, it was revealed that all three of therapeutic music activity interventions had an effect on decrease in unilateral neglect.
