감각운동촉진 훈련이 경직형 뇌성마비 아동의 침흘림 개선에 미치는 영향 (Effects of Sensory-motor Stimulation on the Improvement of Drooling in Children with Spastic Cerebral Palsy)

초록

목적 : 경직형 뇌성마비 아동에게 감각운동촉진 훈련과 키네지오 테이핑을 각각 적용하여 침흘림 개선의 효과와 지속성을 확인하고 근거 기반의 임상적 치료방법으로 제안하고자 하였다.
연구방법 : 2-6세 경직형 뇌성마비 아동 10명을 대상으로 실험군에게는 감각운동촉진 훈련을 대조군에게는 키네지오 테이핑 중재를 실시하였다. 감각운동촉진 훈련 그룹은 30분씩, 주 3회, 12주간 적용하였고 키네지오 테이핑 그룹은 45분씩, 주 3회, 12주간 적용하였다. 침흘림의 변화는 침흘림 정도 및 빈도 척도(Drooling Severity and Frequency Scale; DSFS)와 침흘림 영향 척도(Drooling Impact Scale; DIS)를 사용하여 평가하였다. 평가는 중재 전, 중재 6주차, 중재 12주차, 그리고 추적검사로 총 네 번 측정하였다. 분석방법은 그룹 간 차이를 알아보기 위하여 모수 통계 방법인 독립표본 t-검정(independent-test)을 사용하였다. 그룹 내 중재 전, 중재 6주차, 중재 12주차, 추적 검사 기간에 따른 효과 비교는 모수 통계 방법인 반복측정 분산분석을 사용하였다.
결과 : 두 그룹 간의 비교에서 DSFS의 침흘림 정도와 빈도는 6주차, 12주차, 추적 검사에서 유의한 차이를 나타냈으며(p<0.05), 침흘림 영향에서는 12주차, 추적 검사에서 유의한 차이를 보였다(p<0.05). 그룹 내 비교에서 감각운동촉진 훈련은 시간에 따라 긍정적인 차이가 있었다(p<0.05).
결론 : 감각운동촉진 훈련 중재를 12주 동안 적용한 결과 경직형 뇌성마비 아동의 침흘림 개선에 긍정적인 효과가 있었으며, 추적 검사 결과 중재효과가 지속됨을 확인하였다. 본 연구 결과는 경직형 뇌성마비 아동의 침흘림 중재를 위한 임상적인 근거를 제시하였다는 것에 의의가 있다.

영어초록

Objective : By applying sensorimotor stimulation and kinegio taping of oral sensory motor therapy to children with spastic cerebral palsy, respectively, we tried to confirm the effectiveness and persistence of saliva improvement and propose it as a evidence-based clinical treatment method.
Methods : Sensorimotor stimulation and kinegio taping intervention were divided into two groups in children with spastic cerebral palsy aged 2-6. The sensorimotor stimulation group was applied for 30 minutes, three times a week, for 12 weeks, and the kinegio taping group was applied for 45 minutes, three times a week, for 12 weeks. Changes in acupuncture were evaluated using the Drooling Severity and Frequency Scale (DSFS) and the Drooling Impact Scale (DIS). Evaluation was measured four times before the intervention, week 6 of the intervention, week 12 of the intervention, and follow-up tests. The analysis method used an independent sample t-test, a parameter statistical method, to find out the difference between groups. Repeated means of ANOVA (Repeated means) was used to compare the effects according to the parameter statistical method before the intervention, week 6 of the intervention, week 12 of the intervention, and the follow-up test period within the group.
Results : In the comparison between the two groups, the degree and frequency of drooling in DSFS showed significant differences at weeks 6, 12, and follow-up examinations (p<0.05), and significant differences at weeks 12 in the drooling effect (p<0.05). In the comparison within the group, there were positive differences in sensorimotor stimulation over time (p<0.05).
Conclusion : As a result of applying the sensorimotor stimulation for 12 weeks, it was confirmed that there was a positive effect on the improvement of saliva in children with spastic cerebral palsy, and that the intervention effect continued as a result of follow-up examination.
The results of this study are meaningful in that they presented a clinical basis for the intervention of saliva in children with spastic cerebral palsy.
