의미변이형 원발진행실어증의 언어중재에 대한 문헌고찰 (A Literature Review on Language Interventionfor Semantic Variant Primary Progressive Aphasia)

초록

의미변이형 원발진행실어증(semantic variant primary progressive aphasia, svPPA)은 의미 지식의 손상으로 인해 이름 대기 및 단어 이해의 어려움을 보인다. 기존에는 FTD의 퇴행적 특성으로 인해 언어 중재 효과에 대한 기대가 낮았으나, 최근 svPPA의 어휘-의미 체계 손상에 초점을 맞춘 연구가 증가하고 있다. 본 연구는 svPPA 대상 언어 중재 전략과 효과를 분석하고자 한다. 2010년부터 2025년 1월까지 “PubMed”와 “Google Scholar”에서 "semantic variant primary progressive aphasia" 및 "intervention"을 키워드로 검색하였다. 총 12편의 문헌을 선정하여 1) 연구 동향, 2) 중재의 정량적 요소, 3) 중재 유형, 4) 중재 효과, 5) 중재 접근법을 분석하였다. 중재 연구는 꾸준히 증가하고 있으며, 주 2회 60분간 2-11개월 동안 시행된 경우가 가장 많았다. 모든 연구에서 직접 중재가 이루어졌으며, 대부분 숙제가 포함되었다. 중재 어휘에 대한 개선 효과는 11편, 비중재 어휘에 대한 개선 효과는 5편에서 확인되었다. 중재 접근법으로는 의미 자질 분석, 오류 학습 기제, 그림 보고 따라 말하기/읽기, 개념적 정보 강화 치료 등이 활용되었다. svPPA의 언어 결함은 어휘-의미 처리 손상에서 기인하므로, 핵심 언어 처리 요소를 활용하도록 유도하는 것이 필요하다. 본 연구는 향후 국내 svPPA 중재 연구 설계의 기초 자료로 활용될 수 있다.

영어초록

Semantic variant primary progressive aphasia(svPPA), a clinical syndrome caused by frontotemporal dementia(FTD), is characterized by naming difficulties and deficits in word comprehension. Because svPPA results from the progressive neurodegeneration associated with FTD, expectations for language intervention have been low. However, interventions targeting lexical-semantic deficits have led to more successful outcomes. This review examines language intervention strategies for svPPA and their effects. A literature search was conducted using “PubMed” and “Google Scholar” with the keywords “semantic variant primary progressive aphasia” and “intervention” from 2010 to January 2025. Twelve papers were selected and analyzed based on five criteria: 1) research trends; 2) quantitative components in intervention; 3) intervention type; 4) intervention effectiveness; and 5) intervention approaches. The number of intervention studies has steadily increased—most involved twice-weekly sessions of 60 minutes that took place over two to 11 months. All used direct intervention, often with home practice. Intervention was effective for treated words across all studies and for untreated words in five studies. Additionally, all studies used language expression assessment tools to measure improvement in svPPA. The intervention approaches included those based on semantic feature analysis, error learning mechanisms, repetition and/or reading with pictures, and conceptual enrichment therapy. This review introduced and analyzed various intervention strategies for svPPA. Because svPPA symptoms stem from lexical-semantic processing deficits, interventions should encourage individuals to actively use central components of language processing to strengthen their abilities. These findings provide fundamental data for future research and clinical application of language intervention for svPPA in Korea.
