노인에서의 약물부작용 집중 모니터링 (The intensive monitoring of adverse drug reaction in the elderly)

초록

After domestic spontaneous reporting system was adopted in 1988, our hospital established adverse drug reaction monitoring system within the hospital in 2002. From 2006, our hospital was designated as the ‘Regional Pharmacovigilance Center’and collected the ADR case data from not only the hospital but the local district as well. The result showed that the ADRs case increased from 784 cases in 2006 to 1082 cases in 2008. The local drug monitoring center has expanded to 15 centers nation-wide actively contributing to ADR administration.
According to the cases reported in Korea Food and Drug Administration in 2008, there are 7210 ADR cases and 5.3% of the in-patient is related to ADR. Therefore, in order to use drugs in safety, reporting and collecting data to promote appropriate drug treatment with a strong foundation is necessary.
ADR cases of elderly people and infant is of great significance. Clinical trials for new drugs are performed only for limited age groups and number of people generally excluding elderly people.
However, elderly people have deteriorated organs and strength though aging. Moreover, drug dosage due to basic diseases is 7 drugs in average for elderly people, which is more than other age groups. For these reasons, among total ADR cases, additional study of the elderly ADR cases is performed retrospectively.
This study is based on the 301 ADR cases of patients over 65 years old reported from January 2007 to December 2008. Report rates, types of reported drugs, symptoms, causality assessment and other clinical characteristics are investigated.
This study suggests that monitoring hazardous drugs concerning high ADR possibility for the elderly helps adjust adequate dosage quantity and assists better consulting contribute to secure drug usage for the future.
