지역아동센터 이용 어린이의 비만관리의 한계점과 모바일폰의 잠재적인 활용 가능성: 어린이와 보호자 대상의 질적 연구 (Current Barriers of Obesity Management of Children Using Community Child Care Centers and Potential Possibility of Utilizing Mobile Phones: A Qualitative Study for Children and Caregivers)

초록

본 연구에서는 지역아동센터 이용 어린이의 비만관리의 한계점을 파악하여 이를 극복할 수 있는 모바일폰의 활용 가능성을 제시하고 어린이 비만을 예방 및 관리할 수 있는 효과적인 맞춤형 중재 방법을 도출하고자 하였다. 이를 위해 지역아동센터 이용 어린이, 지역아동센터 선생님 대상 심층인터뷰와 지역아동센터 이용 어린이의 학부모를 대상으로 포커스 그룹 인터뷰를 수행하였다.
연구 참여자는 지역아동센터 이용 어린이와 어린이의 학부모, 지역아동센터 선생님으로, 지역아동센터 이용 어린이20명, 어린이의 학부모 5명, 지역아동센터 선생님 12명으로총 37명이었다. 연구 참여자가 생각하는 지역아동센터 이용어린이의 비만관리 한계점과 이를 해결할 수 있는 모바일폰의 잠재적 활용 가능성 등에 대해 질문하였다.
심층인터뷰 및 포커스 그룹 인터뷰를 통해 자료를 수집하였으며 필사된 질적 자료를 바탕으로 공통적인 내용을 구분하고 키워드를 선택해 분석하였다. 분석 결과, 지역아동센터를 이용하는 어린이의 비만관리 한계점은 자기주도적 비만관리에 대한 동기 결여(만성적인 비만 유발 생활습관, 낙인으로 인한 자신감 부족)와 가정과 지역아동센터의 불충분한지원(맞벌이가정, 센터와 가정 간 식생활 지도 불일치, 반복적인 음식 섭취 강요, 지속적인 영양교육 부재)으로 나타났다.
비만관리 한계점을 극복하기 위해 모바일폰 기반 프로그램을 이용할 경우 시각매체를 활용한 편리하고 재미있는 비만관리 도움(실시간 행동 기록 추적, 게이미피케이션을 이용한 사회적 지지), 환경적 지지(양육자의 돌봄 보완, 네트워크 기반의 지역아동센터와 가정 연계 교육)와 개별화된 중재(개인 맞춤형 점진적인 식습관 변화 유도, 맞춤형 목표 설정) 가 가능할 것으로 나타났다.
지역아동센터 이용 어린이의 주 양육자 부재로 인한 비만관리의 한계점을 해결하기 위해 시공간의 제약 없이 지역아동센터를 비롯한 전문기관의 보호와 관리가 가능한 모바일폰 기반 비만관리 프로그램을 통해 양육자의 부족한 돌봄을충족시킬 수 있을 것으로 사료된다. 또한 어린이의 가정 내식사의 질을 높이기 위해 모바일폰 어플리케이션으로 영양교육과 어린이 맞춤형 식단을 제공함으로써 어린이의 식생활 개선에 도움이 될 것으로 기대된다.
지역아동센터 이용 어린이의 비만관리 프로그램의 효과적인 수행을 위해 어린이 대상 교육과 건강검진을 지원할 수 있는 지역사회의 협조가 필요하며, 모바일폰 어플리케이션과지역아동센터 및 보건의료기관의 연계로 체계적이고 지속가능한 비만관리 프로그램 구축이 가능할 것으로 사료된다.
모바일폰 기반 비만 예방 및 관리 프로그램을 통해 지역아동센터 이용 어린이의 특성을 고려한 맞춤형 중재와 지속적인 피드백을 제공하고, 지역사회의 지원을 바탕으로 어린이양육자의 돌봄 공백을 최소화하여 어린이의 비만관리를 위한 식생활 개선에 일조함으로써 지역아동센터 이용 어린이의 비만관리 한계점을 해결하여 어린이의 생애 전반에 걸친건강관리에 이로울 것으로 사료되며, 궁극적으로 소아청소년 비만율 감소에 기여할 수 있을 것으로 기대한다.

영어초록

Objectives: This study was performed to identify the current barriers of obesity management for children using Community Child Care Centers and their caregivers (parents and teachers working in the Centers). Further, this study explored the possibility of utilizing a mobile phone application for tailored obesity prevention and management programs to overcome the current difficulties associated with children's obesity management.
Methods: The qualitative data were collected through in-depth interviews with 20 obese and overweight children or children who wanted to participate in this study using Community Child Care Centers, 12 teachers working at the Centers, and a focus group interview with five parents of children using the Centers. Data were analyzed with a thematic approach categorizing themes and sub-themes based on the transcripts.
Results: The current barriers of obesity management of obese and overweight children using Community Child Care Centers were lack of self-directed motivation regarding obesity management (chronic obesity-induced lifestyles and reduced self-confidence due to stigma) and lack of support from households and Community Child Care Centers (latchkey child, inconsistency in dietary guidance between the Center and household, repetitive pressure to eat, and absence of regular nutrition education). Mobile phone applications may have potential to overcome the current barriers by providing handy and interesting obesity management based on visual media (real-time tracking of lifestyles using behavior records and social support using gamification), environmental support (supplementation of parental care and network-based education between the Community Child Care Center and household), and individualized intervention (encouragement of tailored and gradual changes in eating habits and tailored goal setting). It is predicted that the real-time mobile phone program will provide information for improving nutritional knowledge and behavioral skills as well as lead to sustainable children’s coping strategies regarding obesity management. In addition, it is expected that environmental factors may be improved by network-based education between the Community Child Care Centers and households using the characteristics of mobile phones, which are free from space and time constraints.
Conclusions: The tailored education program for children using Community Child Care Centers based on mobile phones may prevent and reduce childhood obesity by overcoming the current barriers of obesity management for children, providing environmental and individualized support to promote healthy lifestyles and quality of life in the future.
