캉귈렘의 의철학에 근거한 고혈압에 대한 철학적 반성 (Philosophical Reflection on Hypertension based on Canguilhem’s Philosophy of Medicine)

초록

조르주 캉귈렘(Georges Canguilhem, 1904-1995)은 ‘정상’, ‘병리’, ‘건강’, ‘질병’ 등 의학의 기초 개념을 역사인식론(épistémologie historique)이라는 방법론으로 재정립한 20세기 프랑스의 대표적인 의사학자이며 의철학자이다. 그는 질병에 걸릴 수 있는 가능성이 생명체의 결함이 아니라 생명체로서 인간의 긍정적인 ‘능력’(faculté)이고 본질적인 특성이라고 규정하였다. 따라서 질병은 생명의 또 다른 규범인 것이다. 이러한 ‘질병’ 개념에 비추어 본다면 현대 의학의 고혈압에 대해 새로운 해석과 비판이 가능하다.
본 논문에서는 먼저 통시적 차원에서 ‘고혈압’ 개념이 형성된 과정을 검토하고 고혈압과 심혈관 질환의 역학적 연구에서 밝혀진 복잡한 인과관계를 확인하고자 한다. 다음으로 공시적 차원에서 정상 혈압의 정의와 기준을 검토함으로써 현대 의학의 특징과 한계를 밝히고자 한다. 고혈압에 대한 이러한 공시적이고 통시적인 분석은 고혈압이 과연 질병인지, 더욱이 치료의 대상인지의 문제에 대해 새로운 조망과 시사점을 제시해 줄 것이다. 아울러 현대 의학의 ‘질병’ 개념에 대한 이러한 역사인식론적 분석은 정상과 병리의 전위된 관계, 질병의 규범성, 정상인의 질병 또는 위험 요인의 질병화와 같은 캉귈렘의 핵심적인 사상을 보다 구체적이고 현실적으로 이해할 수 있는 토대가 될 것이다.

영어초록

Georges Canguilhem(1904-1995) is one of the most well-known 20th century French historian and philosopher of medicine, who redefined the basic medical concepts such as the normal, the pathological, health, and disease by historico-epistemological methodology. According to his philosophy of medicine, the possibility of getting ill is rather a positive capacity or fundamental characteristic of human as living being and not its defect. Thus disease is defined as just another norm of living being. Canguilhem’s definition of disease can lead us to new interpretation and criticism upon the definition and the criteria of hypertension in modern medicine.
In order to achieve this goal, firstly this paper aims to trace the formation of the concept of hypertension from the diachronic dimension and then identify complicated cause-and-effect relationships revealed by epidemiological researches on hypertension and cardiovascular disorders. In the second place, analyzing the definition and the criteria of normal blood pressure from the synchronic dimension, modern medicine’s main features as well as its limitations are disclosed. The collaboration of diachronic and synchronic study on hypertension is finally intended to show new perspectives and suggestions on following fundamental problems: is hypertension a disease? if so, is it subject to therapy? Furthermore, the historico-epistemological review of hypertension in modern medicine is expected to allow us a more concrete and practical understanding of Canguilhem’s key ideas, for instance the reversed relationship of the normal and the pathological, the normativity of disease, the disease of normal individual or the transformation of risk factor to disease.
