獨參官桂理中湯의 少陰人 咽喉痛 治驗例 (A Clinical Study of Soeumin Doksamkwankaeijungtang)

초록

1. Background and Purpose I intented to be helpful to understand the consitutional symptoms of soar throat and the application of the prescription through a clinical study of Soeumin Doksamkwankaeijungtang.
2. Methods I studied 16 patients(1 man and 15 women) treated by Doksamkwankaeijungtang in Chewon Oriental Clinic. And then I investigated the characteristics of patients treated by Doksamkwankaeijungtang, remedial values and side effects.
3. Results Doksamkwankaeijungtang was used in the age groups between twenties and sixties. There were 28 kinds of chief complaints including soar throat, tonsilitis, allergic rheum, bitter taste, dry throat, dry cough, stomatitis, lower abdominal pains, climacteric syndrome, etc.
The distributions of remedial values were completely recovered (56.25%), much improved (18.75%), slightly improved(6.25%), no improved(12.5%), aggravated(6.25%), side effects(18.75%). There was one aggravated patient. Using the prescription, I prescribed it without any changing and addition according to the symptoms of soar throat. The remedial value of Doksamkwankaeijungtang was effective and not influenced by the dosage of Ginseng. The onset of action was very quick by the changes of dosage without the relation of age. Side effects were upward tendency of blood presser, headache, chest pain, etc.
4. Conclusion Doksamkwankaeijungtang was specifically effective in soar throat of Soeumin.


1. 연구배경 및 목적少陰人 신정방의 하나인 獨參官桂理中湯의 效能과 活用에 대한 임상적 연구를 통하여 四象體質病證의 理解와 方劑의 運用에 도움을 주고자 한다.
2. 방법1997년 8월부터 2000년 6월까지 본원에 來院한 환자들중 獨參官桂理中湯 5첩이상 투여되고 經過 觀察이 가능했던 咽喉痛을 主訴症으로하는 16명(남자 1명, 여자 15명)을 대상으로 하여 臨床的 有意性을 檢證하였다. 16명은 體幹測定法에 의한 體質鑑別上 少陰人으로 판정한 15명, 太陰人 1명 이었다.
3. 결과獨參官桂理中湯을 少陰人 20대에서 60대에 걸쳐 사용하였으며, 對象患者들의 主訴症은 咽喉痛, 扁桃腺炎, 鼻炎, 口燥, 口苦, 乾咳, 口內炎, 大腸怕寒症, 更年期證候群 등 28종류이었다. 治療效果의 분포는 快癒 56.25%, 良好 18.75%, 好戰 6.25%, 別無效果 12.50%, 惡化 6.25%(태음인의 경우)이었다.
전신증상의 특징으로 舌苔가 黃, 白, 黃黑하게 나타나거나 睡眠障碍, 汗出異常이 있는 경우도 있었다. 處方을 活用할 때는 處方의 변경없이 獨參官桂理中湯만을 사용하였으며 처방내용 중 人蔘의 重量을 20그람 또는 32그람으로 하여 投與하였다. 獨參官桂理中湯의 治療效果는 人蔘의 重量에 관계없이 有效하였으며 效果發顯時間은 年齡에 관계없이 服用量의 變化에 따라 迅速하였다. 副作用으로는 血壓上升, 頭痛, 結胸 등이 있었다.
4. 결론獨參官桂理中湯은 咽喉痛에 特效가 있었다.
