초록

Objectives: This study presents a scoping review of fMRI research on acupuncture for Alzheimer’s disease. It examines commonly used techniques, outcome measures, and identifies research gaps. The review aims to evaluate acupuncture treatment for cognitive impairment through fMRI, highlight methodological issues, and provide recommendations for future research.
Methods: Controlled clinical trials utilizing fMRI in acupuncture treatment for human subjects were selected. A five-stage scoping review framework, based on Arksey and O’Malley’s methodology, was employed. Studies were assessed for their general characteristics, acupuncture methods, and compliance with STRICTA (Standards for Reporting Interventions in Clinical Trials of Acupuncture) guidelines. Results were systematically organized into tables for clarity and reproducibility.
Results: Ninety studies were identified, with 12 meeting the inclusion criteria. Participants included patients with Alzheimer’s, those with mild cognitive impairment, and healthy controls. Cognitive impairment was assessed using tools such as the MMSE and CDR. Common acupuncture points included LR3 and LI4. Electroacupuncture was utilized in five studies, while manual acupuncture was used in seven. The STRICTA evaluation indicated consistent reporting on acupuncture type and rationale, but limited detail regarding practitioner qualifications.
Conclusions: This review reveals significant limitations in fMRI-based acupuncture studies for Alzheimer’s, including insufficient treatment sessions, a lack of disease-specific acupuncture points, few randomized controlled trials, unclear participant selection criteria, and inadequate adherence to STRICTA guidelines. Addressing these gaps with robust study designs is crucial for advancing the field and enabling high-quality reviews.
