Introduction비만과 심혈관계 질환과의 연관성에 대한 연구에 비해 암과 비만과의 관계에 대한 연구는 활발하지 못하다. 하지만 비만은 여러 암의 발생에 영향을 미치며, 유방암과 전립선암, 자궁암, 식도암, 직장암, 방광암 등 여러 종류의 암 발생에 영향을 미친다. 비만이 암을 유발하는 정확한 기작은 아직 밝혀지지 않았지만, 영양과 음식, 그리고 운동 부족 등에 의해서 나타나는 것으로 보인다. 과체중과 비만을 나타내는 수치로 가장 많이 쓰이는 것은 Body Mass Index(BMI)로, 이는 사람의 몸무게를 키의 제곱으로 나눈 수치이다(㎏/㎡). BMI가 18.5 이하일 경우에는 저체중, 18.5∼24.9는 정상, 25.0∼29.9는 과체중, 30이상은 비만이다. 많은 연구결과들에서 지방의 증가에 의해서 직장암, 자궁암, 간암 등 여러 종류의 암발생률과 암 진단 후 사망률이 증가된 것으로 나타났다(Figure1). 암과 비만의 관계에 대한 연구의 길잡이가 되어준 것은 American Cancer society에서 1959년부터 1972년까지 수행된 조사이다. 이 조사결과 체중이 높은 사람들이 그렇지 않은 사람들보다 cancer death가 남자들의 경우 33%, 여자들의 경우에는 55%나 높았다. 이 중에서도 특히, 남자들의 경우에는 직장암과 전립선암이 여자들의 경우에는 방광암과, 유방암, 자궁경부암 등에서 cancer death가 높았다.

Figure2에서 보듯이 미국에서 1960년에서 1980년까지 과체중과 비만인 사람들의 비율은 거의 일정하게 유지되었지만 1980년대와 1990년에 들어서면서 과체중과 비만인 사람들의 비율이 남자와 여자에서 모두 급격히 증가하였다. 2000년까지 미국 성인의 64.5%가 과체중이거나 비만이었고, 이 중에서도4.7%는 BMI가 40이 넘는 병적인 비만 수준이었다.
