Rare but lethal complication of AMI
Increasingly rare but mortality remains high
Epidemiology
Increasing wide-spread use of primary PCI
high usage rate of evidence-based therapies (aspirin, clopidogrel, statins, beta blockers, ACE inhibitors)
- decrease the rate of mechanical complications
Risk Factors
-More likely
older age (>65 years), female sex, prior stroke, CKD
ST segment elevation, initial positive cardiac biomarkers, first myocardial infarction (frequently transmural), severe one-vessel coronary artery disease with lack of collateral formation, cardiogenic shock, cardiac arrest, higher killip class, longer times to first balloon inflation or thrombolytic administration
-Less likely
hypertension, diabetes, prior smoking or prior MI
VSR - Pathophysiology
-Conventional mechanism (sub-acute)
Septum adjacent to the rupture : thin & necrotic
Coagulation necrosis develops within 3-5 days after infarction
